Overview JOB SUMMARY Provides the provision of appropriate nursing support and implementation of evidence-based measures that ensure the well-being of patients receiving care and treatment in the interventional procedural areas and within radiology imaging, which includes adequate and safe monitoring of patients before, during, and after interventional procedures. Interventional Radiology Nurses (IRRN) need to have an understanding of nursing process, providing the standard of care, and guaranteeing quality improvement and exercises influence over departmental activities. MINIMUM QUALIFICATIONS Education: See licenses and certification Experience: Six months experience in critical care or emergency room (within the past three years) preferred. Interventional Radiology nursing experience preferred. Knowledge and Skills: Able to communicate effectively in English both orally and in writing. Able to work and multitask in a fast-pace environment; effectively managing, prioritizing and delegating patient care and completing work assignments and other duties, as assigned. MISSION STATEMENT Telecare will deliver and manage excellent services and systems of care for persons with serious mental illness. POSITION OBJECTIVE The LVN/LPT Supervisor provides supervision to Mental Health Workers in their assigned area, including: hiring, evaluating, scheduling, assigning duties, and overall coordination of client/resident care. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire. Job Description The Medical Coordinator is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $30.00 - $35.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. Schedule is 11pm-7:30am, Sat & Sun. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs.
