Job Summary The Nurse Navigator functions on the multidisciplinary team as an advocate, interpreter, educator, and counselor for patients. Per Diem Primary Function: The RN Care Manager (RN CM) performs utilization management, case management and discharge planning activities for acute care, and/or extended care patients including ambulatory and emergency department patients. Job Summary The Providence Center ACT Engagement and Diversion Team is looking for a motivated, team-oriented Registered Nurse, to help assess and facilitate the tasks and duties listed below for adults with co-occurring conditions, medical complexities, and who are also dealing with unstable living arrangements. Experience with the chronically homeless and addictions populations a plus, and within our new "CCBHC" (Certified Community Behavioral Health Clinic") capacity. Duties & Responsibilities Triage and assess assigned consumers on the team. Assess and evaluate client health care data collected through interviewing, observation, record review and team input. Develop an individualized consumer plan of care. Perform integrated care in accordance with appropriate medical protocol. Identify expected outcomes of care, individualized to the client, with the ultimate goal of improving client’s overall health. Implement the consumer plan of care. Evaluate the consumer's response to the plan of care and update the plan based on consumer's response. Provide support to consumers regarding daily living issues, conducting community and home visits as necessary. Instruct consumers and families concerning the disease process, plan of treatment and rehabilitation course, using available teaching materials when appropriate. Educate consumers on the various medications on the expected effects and side effects. Question any aspect of the consumer's treatment regimen which endangers the level of wellness. Administer medications as per prescriber orders (Injections and PO). Implement appropriately prescribed medical regimen while following the 6 rights of medication administration. Monitor lab results and complete related documentation Ensure that externally prescribed medications (from outside providers i.e. PCP) are accurately documented in EMR. Collaborate with the staff members in providing care to consumers. Perform recordkeeping in accordance with Agency requirements. Complete initial and annual assessment forms. Responsible for the overall coordination of the Plan of Care and is proficient in all components of the nursing process. Directly responsible to the Clinical Supervisor. Registered Nurse (RN) Job Responsibilities: Provides skilled nursing care to patients in their homes (including admission/discharge) on a family centered basis while recording/maintaining complete patient record on Portable Electronic Device and/or paper record. Teaches, demonstrates, supervises and evaluates care given by peers, HHAs, family members and significant others. According to age and developmental level of the patient, systematically assesses the patient's physical, psychosocial, emotional, cultural, spiritual and discharge needs. Develops and implements a plan of care for each patient. Utilizes effective oral and written communication skills to include completeness and accuracy of documentation. Evaluates the plan of care and reformulates th goals as necessary. Demonstrates accurate technical skill and operational knowledge of patient care equipment and procedure, including venipuncture to administer IV medication, TPN, lipids, etc. Recognizes unsafe and life-threatening situations and intervenes appropriately. Registered Nurse (RN) Education and Experience Requirements: Graduate of an accredited school of nursing is required Registered Nurse (RN) license issued by the state of Connecticut is required Two years of recent medical/acute care or critical care experience as a registered nurse, or an equivalent level of experience preferred Achieve Home Health Care specific competencies within the time frame determined by the department Valid Connecticut Driver’s License and proof of current insurance coverage required Why Choose Day Kimball Health?
