Responsible for all Performance Improvement and Education activities/programs within the entity's Emergency Departments. Assures department is competitive based on evidence based practice, quality outcomes & efficiency of care. Performs a leadership role and collaborates with other Hospital leaders to establish and implement department policies and practices. Maintains environment of care (EOC) readiness in the department. Reports and follows-up on inquiries, equipment problems and EOC issues. The Medical Assistant administers and performs triage and direct care to assigned patients under the direction and supervision of the RN/Nurse Manager. This includes functions such as height/weight and vital signs, updating medical records, as well as assisting the clinicians with exams as requested. Also ensures a smooth flow of patients through the office by providing efficient basic care to patients. The Medical Assistant performs routine administrative and clinical tasks to keep the office running smoothly. The Medical Assistant also performs secretarial support for the office, assist the front desk operations in performing general clerical tasks, such as checking patients in and out, filing, copying, answering the phone, and related duties as requested by the Practice Manager. The certified diabetes care and education specialist delivers comprehensive seamless educational support svcs to patients and their families/care partners to bridge the gap and integrate clinical/self-mgmt. aspects of diabetes. Provides collaborative comprehensive person-centered care/education conducive to behavior change and improved quality of life across the lifespan. Supports/advocates for those affected by diabetes to optimize quality care. Promotes self-mgmt. to achieve individualized behavioral/treatment goals that reduce risks and optimize health outcomes. Participates in case mgmt, patient care planning, goal setting, and development and maintenance of education curricula/materials. Assists the Education Coord in assessing, planning, implementation and evaluation of Patient Centered Diabetes Educ program. Collaborates with other educators identifying the needs, developing/providing clinical programs and services. Promotes and disseminates diabetes information to professionals and public following established American Diabetes Association guidelines. Educator Credentialing as Diabetes Care and Education Specialist required within 1 year of hire. Must maintain 15 diabetes related CEU credits per year and active registration and licensure.
