Responsibilities: • Provide supportive services to nurses and other staff members as needed and perform assigned duties. • Assist residents with activities of daily living in accordance with care plans and established policies and procedures. • Assist residents with lifting, turning, moving, positioning, and transportation to and from beds, chairs, bathtubs, wheelchairs, lifts, and other equipment. • Coordinate dining room services during assigned mealtimes, including setup, cleanup, meal tray delivery, feeding assistance, and documentation of meal intake. • Deliver nutritional supplements to residents at assigned times and provide assistance as necessary to promote proper intake. • Document nutritional and fluid intake accurately. • Assist with weighing residents according to facility policy and record weights in designated locations. • Report significant weight changes to the nurse and supervisor. • Monitor and track residents' skin condition and report pressure areas, skin breakdown, or skin tears to the nurse and supervisor. • Assist nursing staff with toileting program activities. • Complete daily flow sheets to document that assigned tasks have been completed. • Document resident progress as directed by the supervisor. • Keep the nurse and supervisor informed of factors that may interfere with completing assigned duties, including resident refusal or reassignment to another nursing unit. • Promptly report changes in a resident's condition to the resident's nurse and supervisor. • Maintain a safe, clean, and comfortable environment for residents. Qualifications: • Licensed Nursing Assistant certification in good standing with the state. • High school diploma or equivalent. • Current CPR/BLS certification. • Must have no disciplinary action in effect against certification. • At least one year of experience as a Certified Nursing Assistant in long-term care. • Ability to communicate effectively with residents, families, nurses, supervisors, and other members of the care team. • Compassionate, dependable, and committed to providing high-quality resident care. • Ability to safely assist residents with mobility, personal care, and activities of daily living.
