Job Overview The primary function of the Home Health Infusion Nurse is to provide exceptional in-home IV infusion therapy, nursing care, and response management. Partner with the patient, family and care team in developing and implementing plan of care with specific goals under the direction of the physician. Initiate and coordinate care conferences with the participation of all members of the care team as needed. Provide education and support to patients and caregivers that enable them to safely and effectively self-administer medications and solutions. This includes storage, aseptic technique, equipment use, side effects and potential complications. Coordinate the discharge of patients when goals are met with the physician and other disciplines to achieve an orderly transition for the patient/family.
