Description The Per Diem Assistant Nurse Manager for Labor & Deliver is a Registered Professional Nurse with broad knowledge and skills who acts as clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager on a per diem basis. Responsibilities Patient Care 1.Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2.Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3.Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4.Validates staff members round hourly on all patients by direct observation and patient feedback. 5.Assesses staff competency in PHR and provides consistent meaningful feedback with more focused feedback as needed. 6.Conducts rounding of all patients on a daily basis (Monday-Friday.) 7.Communicates quantitative and qualitative information regarding the patient experience and staff engagement to the staff and the Interdisciplinary Patient Care Team that leads to the development and implementation of an action plan. 8.Collaborates with the physician dyad and escalates concerns as appropriate. 9.Follows up on all patient concerns in a timely manner and provides service recovery as needed. 10.Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. Overview VNS Health Psychiatric RN provides mental health assessment of and treatment to patients with psychiatric symptoms, cognitive deficits, and/or behavioral problems. Description The Assistant Nurse Manager for the Emergency Department is a Registered Professional Nurse with broad knowledge and skills who assumes the role of a clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager. Requirements: Primary Care Nurse Practitioner serves as a clinical representative providing comprehensive, compassionate, and evidence-based primary care services to individuals residing in County Correctional Facility and County Shelter. Under the direction of BNBMC Occupational Health Medical Director and County Facility Leadership the Nurse Practitioner exercise sound clinical judgement and advocate for health and safety of patients while balancing operational requirements of the correctional and shelter environments. The primary care nurse practitioner (PCNP) renders medical care to patients in the ambulatory clinics and off-site practices. The PCNP is responsible for providing comprehensive care management, chronic disease management, preventative care and wellness. The nursing process will be utilized (assessment, planning, intervention, evaluation, and teaching) in providing care. Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrate knowledge of the principles of growth and development and possess the ability to assess data reflective of the patient’s status. Completes assessments including history and physical, medication reconciliation, problem list and develops a plan of care. Performs assessment and plans for ongoing treatment for patients in the service. Orders lab and diagnostic tests for patients as appropriate to formulate diagnosis, develop treatment plans. Reviews results of lab and diagnostic tests and makes adjustment to the treatment plan accordingly. Prescribes medication appropriate for the condition.
