This position collaborates with multidiscipline in order to optimally prepare the patient for their surgery/procedure. Provide compassionate, professional service that promotes a positive patient experience. Clinical Procedures: Administer oral and injectable medications, immunizations, and treatments as directed by the provider and permitted by state regulations. Assist with wound care, dressing changes, bandaging, irrigation of eyes and ears, and other clinical procedures. Initiate and monitor IV infusions when appropriately trained and permitted by state regulations. Perform phlebotomy and prepare specimens for laboratory testing. Perform CLIA-waived laboratory testing in accordance with established procedures. Conduct in-office diagnostic testing, including but not limited to: Electrocardiograms (EKGs) Vision screenings Pulse oximetry Peak flow testing Oxygen administration as ordered Inhalation treatments Spirometry Perform PT/INR testing using approved equipment (CoaguChek/Coag-Sense) and glucose testing using a glucometer. Complete laboratory quality controls and maintain required testing logs. Clinical Documentation & Care Coordination: Accurately document patient encounters, treatments, diagnostic results, procedures, supplies utilized, and telephone encounters within the EMR. Complete outbound and inbound patient communications, including: Appointment reminders Medication reminders Transitional Care Management (TCM) — Medicare-supported outreach within 2 days of hospital or facility discharge to reduce readmissions and support safe care transitions Preventive care outreach Support Annual Wellness Visits (AWVs) by preparing Health Risk Assessments (HRAs), updating preventive care records, and ensuring completion of required documentation. Assist in patient outreach, scheduling of preventive screenings, and documentation of completed services. Care coordination activities Maintain patient medical records in accordance with organizational policies and HIPAA requirements. Administrative & Operational Support: Professionally manage front office responsibilities, including: Answering incoming telephone calls Greeting patients Scheduling appointments Patient registration and check-in/check-out Processing referrals Prescription refill coordination Routing clinical and administrative inquiries Monitor inventory levels of medical supplies, pharmaceuticals, and disposable clinical items, and assist with ordering as needed. Summary for Certified Nursing Assistant (CNA) Perform direct resident care duties under the supervision of licensed nursing personnel. Assist with promoting a compassionate physical and psychosocial environment for the residents. Responsibilities of Certified Nursing Assistant Provide personal care (i.e., grooming, bathing, dressing, oral care, etc.) of residents daily and as needed. Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis. Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics. Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned. Routinely turn and position residents, as necessary. Observe residents carefully and report changes in condition to Charge Nurse. Participate in collecting data needed for the accurate completion of the MDS and the plan of care. Provide input to the nurses for development of the plan of care. Provide adequate hydration to residents at regular intervals each day according to specific resident needs. Prepare residents for each meal and transport to dining room (unless a bed patient) making sure that food tray is accessible, self-help devices are available (if needed), and assistance is provided to those residents who cannot feed themselves. Keep an accurate record of food intake and output, as directed. Be alert for proper ventilation, temperature, light and noise control. Take TPR (temperature, pulse, & rate), blood pressure, weighing of residents, etc., as directed. Promptly answer call lights and assist with other resident needs. Assist in the orientation and training of other staff, as requested. Requirements/Qualifications of Certified Nursing Assistant: Current Nursing Assistant certification from state of Florida. Current/active CPR certification, preferred. At least one (1) year Skilled Nursing experience preferred. Effective verbal and written English communication skills. Highest level of professionalism with the ability to maintain confidentiality.
