Participate in care gap closure efforts by documenting and assisting with HEDIS measures (PHQ-9, COA, fall risk, cancer screenings, A1c, etc.). Care Coordination & Patient Engagement: Assist with appointment confirmations and follow scheduling protocols. Support transportation scheduling when applicable. Promote and assist with Patient Portal enrollment. Make outreach calls for wellness checks, emergency preparedness, or appointment rescheduling. Serve as a compassionate advocate for patients, helping them navigate care and recoveries post-hospitalization or urgent care visits. Collaborate with front desk and clinical teams to ensure smooth clinic flow. Operational & Systems Support: Clear telephone encounters and respond promptly to clinical messages. Maintain up-to-date e-filing of documentation (within 48 hours). Monitor faxes and EMR “Jellybeans” and task buckets for timely action. Utilize tools such as Availity, state immunization portals, lab portals (LabCorp, Quest), diagnostic/specialist portals, and internal ticketing systems (Cano.Net). Understand and support prior authorization processes. Education & Experience High School Diploma or equivalent. Job Description Nemours is seeking a Care Coordinator RN for our Cardiology team in Jacksonville, FL. The Cardiac Center High Risk Nurse (RN) Coordinator is responsible for providing specialized professional nursing care to patients with complex congenital heart disease by providing direct and indirect care in collaboration with the physicians, advanced practice providers, nursing, social workers, genetic counselors, nutrition services and other clinical staff including but not limited to admission and discharge planning of all patients admitted to the comprehensive cardiac care unit. This position will work in a multidisciplinary team to innovate, improve, and advance the quality of care through a patient centered approach. Essential Functions Delivers care in a nondiscriminatory manner that is sensitive to and values diversity and serves as a liaison between patients, their families, and members of the healthcare team. Provides chronic care management services on a regular basis to assess patient compliance to care plan, health status by completing a nursing assessment; Performs telephone triage, explains, and confirms understanding of prescribed treatment regimens/plan of care and confirms compliance to same, notifies appropriate physician and/or advanced practice provider of abnormal findings and coordinates with other disciplines to ensure patients receive appropriate and timely care. Attends daily surgical rounds, weekly case conference, discharge planning conference and oversees the execution of the discharge plan. Assists Clinical Psychologist in addressing patient and family needs while in the hospital and post discharge. Participates in quality initiatives, process improvements, and research activities to improve patient outcomes, patient experience, and patient safety. Responsibilities : Responsible for assisting patients with and per plan of care, the following: Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency) Safe transfer and ambulation techniques Exercise according to the plan of care Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency) Patient’s laundry Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.) Observation and teaching of home safety practices Recognition of an emergency situation and knowledge of emergency management procedures Must follow patients’ plan of care and complete all tasks assigned Communicates in a constant, clear, and effective way with the client, family/caregiver and office Observes and documents patient condition and reports any changes, to the assigned nurse supervisor Requirements : Meets all training requirements in accordance with state and federal laws when applicable Ability to read, follow written instructions and document care given Possess and maintains current CPR certification in accordance with state regulation PPD or chest x-ray if applicable Successful completion of Maxim screening examinations Participation and completion of Maxim’s competency program Must meet all federal, state, and local requirements The Lakes benefits : part-time position.
