Administer medications, monitor vital signs, and evaluate patient and family needs. Assist with plan of care preparation and execution, including infection control and symptom management. Participate in bereavement support and may assist with death pronouncements as allowed by state law. Provide guidance to hospice aides under RN supervision as per Nurse Practice Act regulations. Participate in call schedules and provide on-call nursing support as directed. About You Qualifications Licensed Practical/Vocational Nurse (LPN/LVN) licensed in the state of employment. CPR certification required. Valid driver’s license and current automobile insurance. Graduate of an accredited Practical Nursing program. Minimum of 2 years’ experience as an LPN/LVN in hospice, home health, or long-term care; 3–5 years preferred. Work Environment Care is provided in patient homes or institutional settings with travel required. Exposure to infectious diseases, bodily fluids, and hazardous materials possible. (FL Only) Certified Medical Assistant preferred OR the ability to obtain the certification within the first 120 days of employment. To be eligible for employment and to obtain an MA certification, you must be a graduate of a nationally accredited Medical Assistant program OR served in the Armed Forces as a Medical Technician with honorable discharge. Current CPR and/or BLS certification preferred OR the ability to obtain the certification within 30 days of employment via Women’s Care training. Experience working in OBGYN or Primary Care a plus. Working knowledge of medical terminology. Ability to perform phlebotomy and administer injections, if applicable. Ability to perform dosage calculations and measurement conversions. Must possess the ability to interact and communicate professionally and effectively with all levels of internal personnel, providers, personnel of outside organizations, and patients and their family members, both written and verbally. Must possess good organizational skills, handle multiple tasks concurrently, and be able to work effectively with all levels of aforementioned groups. Ability to use a computer and enter data in electronic medical records application, preferably eCW. Demonstrated ability to work in a fast-paced, deadline-driven environment.
