The Concierge is responsible for a variety of clinical functions, including but not limited to assisting the provider with care and treatment, laboratory testing, x-ray examinations, and other duties. The Concierge also performs patient registration, collection of payments, rooming, and patient discharge. All Concierges are expected to maintain competency and perform in all assigned positions on a rotating basis. This position is a vital part of the urgent care team that works under the direction of the Site Manager and works closely with the provider and other clinical staff. Must demonstrate discretion; good judgment; diplomacy; decision-making; and a customer service attitude. Collects identification and payor related documentation, conducts registration using EPIC workflow. Measures and documents vitals and communicates the resulting data to the consumer and provider, including temperature, pulse, oxygen saturation, respiration, blood pressure, height, weight, and pain level. Documents chief complaint, allergies, health history, and current medications accurately in EPIC for review by provider. Performs and documents laboratory testing and collections per provider orders and/or standard operating procedures, to include venipuncture with prior venipuncture competency check-off. Administers and documents oral and intramuscular medication using the appropriate medication verification workflow. If a medical assistant, then may perform injections with the appropriate competency checkoff Assists provider with procedures to include, but not limited to maintaining sterile field, laceration repair, I&D, and application of ortho-goods and assists with ortho-glass application. Performs diagnostic x-rays and documents completion of radiology examinations within the licensed scope of practice as ordered and verified by the provider. Performs x-rays following established protocols and utilizes appropriate exposure factors for care and safety. Ensures images and applicable communication are transferred to radiology group for overread and assures all images have a report on file in the medical record. Behave in a professional and empathetic manner to build trust, comfort, understanding, and relationships with consumers and secondary guest(s) and in-line with patient experience standards. Welcomes the consumer and secondary guest(s) to the center (including eye contact, smile, greeting, and personal introduction) and inquires about the purpose and nature of the visit. Recognizes and notifies provider immediately of emergent patients, performs BLS, provides oxygen administration, and assists provider with emergency care. Schedules primary care referrals for consumers Maintains office supplies, medical supplies, and medications at appropriate par levels as assigned. The MDS Coordinator will be responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc. With direction from the Director of Nursing and VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department. Responsibilities • Works in collaboration with the Interdisciplinary Team to assess the needs of the resident; Provides interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. • Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS, assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. • Assist with coordination and management of the daily stand up meeting, to include review of resident care and the setting of the assessment reference date(s). • Complies with federal and state regulations regarding completion and coordination of the RAI process. • Monitors MDS and care plan documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Maintains current MDS status of assigned residents according to state and federal guidelines. • Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Qualifications • Graduate of an approved RN / LVN program and licensed in the state of practice, required. • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. • Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. • Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. • Knowledge of the care planning process. • Experience with MDS 3.0, preferred.
