Recording vital signs, including blood pressure, pulse, temperature, and respiration rate, as requested by staff. In collaboration with physician, the NP will provide comprehensive assessment, including multifactorial complexphysical examination and psychosocial needs, self - management education and tools to better manage patient’s multiple conditions. As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primary care services to an assigned the at risk panel of Elderplan members who reside in the community. The NP will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives. The NP will act as clinical resource to the member, family and interdisciplinary team.
