Schedule: Every Saturday & Sunday Coverage Area: San Mateo and Santa Clara County Essential Duties & Responsibilities include (but are not limited to): Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations. Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care. Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family. Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues. Education/Experience : One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing. Experience in acute care/ home health and/or hospice setting preferred . License: Current license as a registered professional nurse in the State of California or Licensed Vocational Nurse in the State of California

Admission nurses are responsible for orienting new patients to the hospice program, assisting the patient and caregiver with all the legal paperwork, and completing a thorough nursing assessment. The candidate is required to work 10-hour shifts, 4 days a week (Wednesday to Saturday 2PM - 12:30AM) Candidate must be willing to travel across the Bay Area (Up to 150 miles per shift) Qualifications The candidate must be willing to travel throughout the Bay Area. This position involves regular driving over long distances, so comfort with travel is essential Minimum 1 year of hospice experience, SNF, or hospital experience Ability to establish both hospice eligibility and diagnosis when patient is appropriate Willingness to travel to meet our patients where they live (private homes, assisted living facilities, hospitals, etc.) Reliable transportation, valid driver’s license, and auto insurance Registered Nursing Licensure in the state of California

POSITION DESCRIPTION: A licensed professional nurse immediately under the Director of Nurses. GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Assist the Director of Nurses as she/he instructs in areas of personnel, orientation, supervision, and staffing. • Periodically checks residents’ records and reports. • Keeps the Director informed of potential problems or problem areas within the nursing department with accurate reporting. • Reviews physician’s orders, transfer and admission information for completeness. • Review transcriptions of physician’s orders to resident charts, with medication and treatment sheets to assure that they are accurate and current. • Assist in admitting, transferring and discharging residents. • Make weekly skin assessment rounds with the Treatment Nurse. • Make Licensed Nurses’ monthly schedule. • Work with the consultants as necessary and implement recommended changes as directed by the Director of Nurses. • Review, update, and revise resident Plans of Care for appropriate resident goals, problems and approaches to assure that appropriate nursing care is being rendered. • Assure that nursing personnel follow established infection control and aseptic techniques when isolation procedures become necessary. • Review resident charts for completeness and appropriateness of treatment and medication orders. • Review restraint assessments, pain assessments, oral assessments, psychotropic assessments, and bowel and bladder assessments for completeness. • Check completeness and accuracy of Intake and Output documentation. • Participate in Weight Variance and Pressure Ulcer committee meetings. • Attend staff meetings and in-service classes. • Review monthly recapped physician’s orders with medication sheet, one week before the end of each month to ensure the accuracy in the transcription of physician’s orders. • Others as may become necessary and appropriate or as may be directed by the Director of Nurses or the Administrator. SUPERVISION • Is alert to efficient functioning of nursing department with other facility departments. • Periodically makes nursing rounds. • Assure that nursing personnel are providing adequate nursing care in accordance with established nursing service procedure. • Follow up on correction of findings and deficiencies of Department of Health Services, Consultants, and Quality Assurance Team. • Review and evaluate nursing personnel and make recommendations to the Director of Nurses. • Assist in orientation of newly assigned personnel as well as supervise and instruct other nursing personnel. • Assist in supervision and direction of nursing personnel. • Assist in investigation of unusual occurrence or alleged abuse and promptly notify the Director of Nurses. Duties of Hospice Registered Nurse Include: Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness. Conducts home visits and assesses/evaluates patient's status. Initiates the plan of care and makes necessary revisions as patient status and needs change. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Administers medications and treatments as prescribed by the physician. Attends and participates in Interdisciplinary Group Meetings. Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Supervises ancillary personnel and delegates responsibilities when required. Qualifications: Graduate of accredited school of nursing Current RN license in California Current CPR Certificate Registered nurses shall have a minimum of (a) one year of experience as a professional nurse within the last three years OR (b) have a baccalaureate degree in nursing from a program accredited by the National League for Nursing and current RN license. Minimum of two years experience, at least one of which is in the area of public, home care, or hospice nursing. Must have a valid CA driver license and an automobile that is insured in accordance with the requirements of state of California and is in good working order.
