Advanced Practice Registered Nurse (APRN) Landmark Hospital of Columbia is searching for a Nurse Practitioner to join our dedicated and professional Hospitalist team. We are a private, small LTAC hospital where we are truly one team working together in a positive environment. With an average length of stay of 25-30 days, we have the privilege of building a more meaningful connection with our patients and actually seeing them recover. Summary : The Nurse Practitioner is responsible for assessing, planning, implementing, evaluating, and documenting care activities in accordance with hospital standards in order to deliver efficient, effective, and quality patient care. Must be able to demonstrate the knowledge and skills to provide care appropriate to the age of patients served. Responsibilities : In this role, the Advanced Practice Registered Nurse (APRN) performs at an advanced practice level that will include, but not be limited to: • Providing direct Nursing care to a selected population of patients • Provides advanced nursing practice care to critically ill and medically complex patients • Manages the care of patients with medical complex problems that require advanced nursing skills • Enhancing patient care by integrating the competencies of clinical practice, education, consultation, and research The APRN accepts responsibility, accountability, and obligation to practice in accordance with usual and customary advanced practice nursing standards and functions as defined by the State Board of Nursing and in compliance with the Collaborative Practice Agreement. In this role, the ARNP is accountable for assessing, planning, implementing, evaluating, and documenting care activities in accordance with hospital standards to deliver efficient, effective, and high-quality patient care. The APRN must be able to demonstrate the knowledge and skills to provide care appropriate to the age of patients served. Evaluate the client/patient’s physical condition and the need for services. Complete required screenings, record the client/patient’s initial medical histories and vital signs; coordinate any required action planning based on results. Monitor the health status of the client/patient during services provided. Work with the treatment team to coordinate care based on client/patient needs. Create action plans for client/patient based on physical and mental health in regards to treatment needs. Oversee infectious disease control, client/patient hygiene, and related client & facility health issues, under the supervision of nursing leadership. Provide disease prevention, risk reduction and other health education as needed. Arrange medical appointments for client/patients dealing with medical conditions that occur during treatment and manage medical emergencies, such as accidental injuries and/or drug reactions. Consult with a physician/advanced practice provider as necessary or advocate for medical services through managed care organizations. Arrange or monitor special dietary needs for medical conditions. Review medication requirements with clients/patients, educating the client and guardian (if necessary) about the benefits of taking prescribed medications as prescribed. Administer and oversee administration of medications via various methods according to service line requirements; Monitor medication compliance in regard to State Regulatory standards. Consult with the physician/advanced practice provider or pharmacy to confirm medications prescribed. Oversee therapeutic injection of medication (subcutaneous or intramuscular). Monitor lab levels including consultation with physicians/advanced practice providers, clients, and clinical staff. Coordinate medication needs with pharmacies, clients/patients, and families, including the use of indigent drug programs. Monitor medication side-effects including the use of standardized evaluations. Monitor physician orders for treatment modifications requiring client/patient education. Assist Physician/Advanced Practice Provider with client/patient exams. Set up and draw any necessary labs and perform any special procedures as requested by the Physician/Advanced Practice Provider. Complete all required documentation in a timely manner as indicated in company policies and procedures. Demonstrate interest in long term and short term goals and objectives of the company. Other duties as assigned by program leadership. Qualifications: Current Registered Nurse or Licensed Practical Nurse licensed in the state of practice Must be willing to become licensed in additional states, other than the state currently licensed in. Must be willing and comfortable with providing telehealth services to the individuals we serve. FQHC: BCLS certification required, ACLS certification preferred. Education: Successful completion of background check including criminal record, driving record and abuse/neglect. Completion of New Hire Orientation at the beginning of employment. All training requirements including Relias Learning at the beginning of employment and annually thereafter. Annual maintenance of licenses, and certifications. If transporting clients the following will be required: Valid driver’s license within state of residency Verification of Limits of Liability on personal car insurance within thirty (30) days of employment and annually thereafter Physical Requirements: Sedentary work. Sedentary work involves lifting no more than 10 pounds at a time and occasionally lifting or carrying articles like docket files, ledgers, and small tools.
