The wound care nurse assess, treat and create care plans for patients with complex wounds, ostomies, and or continence conditions. Cleaning surgical wound and helping a patient with pain management, and consulting staff on how to prevent pressure ulcers. Identifies, manages, and treats specific skin conditions, such as pressure injuries, diabetic, venous, or arterial ulcers, and traumatic or complicated wounds. Provide wound care to assigned residents in accordance with physician orders, following manufacturer recommendations and appropriate techniques. Completes a thorough and accurate wound assessment upon notification of wound. Completes follow-up assessments weekly and as needed. Documents all assessments in the medical record. Confers with the care plan team in developing the comprehensive care plan. Documents resident care problems, abnormal findings, and changes in resident’s condition. Report to physician, supervisor, and resident/representative. Monitors implementing preventive measures such as turning/positioning, pressure relief devices, nutrition/hydration measures, and toileting/incontinence care. Coordinates and ensure completion of the state required Minimum Data set on all residents throughout the facility to include admissions, significant changes, quarterly and Medicare assessments in a timely manner. He/she identifies resident problems from the MDS and other assessments and develops the initial individual Care Plan for each resident. The MDS Coordinator reviews and optimizes the MDS Process to ensure appropriate services are rendered justifies facility reimbursement.Qualifications: Current Georgia Nursing Licensure, LPN or RN Experience in clinical and utilization experience in a Long Term Care or skilled facility Strong working knowledge of Medicare, Medicaid reimbursement guidelines, PPS and Rugs categories as it relates to the MDS process. Licensed Practical Nurse DUTIES: Recording health details of Residents; including vitals & temperature. Administering medications and injections to Residents as needed. Treating and dressing wounds and bedsores as needed. May be required to supervise Certified Nursing Assistants (CNAs). Helps Residents get dressed & take care of personal hygiene. Monitors Residents’ food and liquid intake and output. REQUIREMENTS: Must be able to work as a team member. Valid GA License and be in good standing with State Registry.

Assist with the supervision of Nursing Assistants Admit, transfer and discharge residents as required Perform ongoing assessments of residents Consult with physician in providing care, treatment, diagnostic and therapeutic services. Notify physician of residents incidents/accidents, changes in residents condition, lab work, etc and obtain orders as indicated. Transcribe physician orders per policy and provide follow through as ordered. Administer Medications and treatments in accordance with state guidelines, as ordered by the physician and observe for possible adverse reactions. Administer professional services and provide direct nursing care as necessary.
