The home health registered nurse uses the nursing process (assesses, plans, implements, evaluates) to provide patient care in the home setting. Provides individualized patient care for patients in all developmental stages throughout the life span including: Neonates - 0-28 days, Infant - 1-12 months, Child - 1-12 years, Adolescent - 13-17 years, Adult - 18-72 years, Geriatric - 72 + years, according to established policies, procedures, guidelines and nursing standards of care. Education: Graduate of an accredited or approved school of nursing, either an AD, Diploma, or BSN program. 2. Experience: One year RN experience and a total of 2 or more years clinical experience is required. Supplemental experience may include experience as LPN, CNA, military medic, EMT or related experience. Home health experience preferred. Less than 1 year RN experience requires 1 year of clinical experience as LPN (Internal use only). Therapy Assistants (PTA, OTA) with 1 year of Home Health experience and at least 6 months RN experience (internal use only). 4. Essential Technical / Motor Skills: Hand/eye coordination in order to give injections, use computer, etc. Must be able to communicate and be literate in the English language. Able to manipulate patient care equipment, to properly transfer and guard patients. 5. Interpersonal Skills: Ability to develop positive interaction with patients, patients’ families, physicians and staff in order to effectively care for the patients. 6. Essential Physical Requirements: Ability to transfer and/or maneuver objects weighing at least 50 pounds in the assessment and implementation of patient care. Requires frequent pushing, moving, lifting of patients. Positioning of patients, giving patients baths and ambulating patients expending much physical effort. Utilize hearing to auscultate lung sounds, bowel sounds, hear alarms, and effectively communicate with patients, families, physician, and staff. 9. Exposure to Hazards: Noise, exposure to blood borne pathogens and body fluids, infectious diseases, and needle puncture wounds. May be exposed to dangerous animals and traffic hazards while home visiting. May encounter patients and other situations which present a potential threat to personal safety. Documents all activities/interventions, patient/family responses, medication prescribed, etc., in the medical record. Communicates with patients regarding provider advice/instructions within scope of training. Adheres to infection control/safety guidelines, and confidentiality policies. Phones in prescriptions to pharmacies based upon provider’s written orders. Provides results and other information to patients as specifically directed by provider. Assists patient with the scheduling of future appointments, surgeries, and/or diagnostic testing, as necessary. The Medical Assistant is not authorized to interpret reports or results or independently dispense medical advice to patients. Performs other duties as assigned. Qualifications: Must be certified Medical Assistant. In lieu of certification at hire, must have successfully completed a medical assistant training program or have two years of experience as a Medical Assistant (eq. Army Medic, Navy Corpsman) at hire. For those that have completed a medical assistant training program or have two years of experience, must obtain Medical Assistant certification within 18 months of hire. Incumbents hired before March 2021 may be considered grandfathered in terms of education. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. Assist with rescheduled, missed, or reassigned visit requests based on triage direction. Participate in calendar and scheduling modifications, including service codes and clinician assignments. Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection. Assist with TPE reviews, QAPI efforts, and regulatory compliance. Ensure infection control standards and universal precautions are consistently followed. Provide leadership and guidance to field staff; conduct supervisory visits when appropriate.
