QUALIFICATIONS • Current nursing license in the state in which practicing. • Strong organizational and mathematical skills. • Strong verbal and written communication skills. • Previous experience in long-term resident care Medicaid reimbursement, PPS, MDS and QI.Experience as a Director of Nursing in long-term care preferred. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care.
