We are seeking a full-time Nurse Case Manager to cover the Minneapolis / St. Paul area. This position is responsible for the consultation, development, and implementation of the rehabilitation plan. A Nurse Case Manager works independently using their own discretion to assist injured workers/consumers in their recovery so that they may return to work. The Nurse Case Manager works with all parties to the claim including: the injured person, the claim’s examiner, employer, attorneys (plaintiff and defense) and medical providers. Why Paradigm? Autonomy to manage your own schedule. Responsibilities: Accepts cases for referral and management in accordance with Minnesota Rules. Counsel, direct and motivate consumers to reach their highest level of function. Establishes relationships and maintains communications with consumers, families, providers, Paradigm’s carrier clients and employers. Ensures that all communication is culturally sensitive and meets requirements to maintain injured worker confidentiality. Completes accurate and timely written reports in accordance with Minnesota rule and documentation standards. Ensures that rehabilitation plan is implemented and documents progress towards achievement of the rehabilitation plan. Assist sales/marketing team as needed in order to build caseload. Develop solid relationships with customers in order to assure repeat business. Qualifications: Current, unencumbered Registered Nurse (RN) license in MN Registered Minnesota Qualified Rehabilitation Consultant (QRC) preferred. Candidates who are not yet registered must complete a Minnesota QRC internship, provided by Paradigm. Case Managers may also need regional or national certifications and must adhere to the standards of practice outlined by the specific national certifying body or the jurisdictionally accepted standards of practice. Two years of full-time equivalent direct clinical experience to the consumer (required for casework referred from Accounts requiring accredited case management). Professional licenses or certifications required to meet qualifications for this position must be current, unrestricted and allow for practice within a state or territory of the United States. Valid driver’s license, and good driving history Competencies: Exceptional time management and organization skills are required. A full-time QRC is responsible for approximately 25 files that all require contact and reporting within specific timelines. The ability to communicate effectively with a wide range of people on a daily basis utilizing both verbal and written communication. Flexibility, versatility, and problem solving – this role works at a fast pace. Technology proficiency – QRC’s at Paradigm CCS use an advanced Case Management software solution to do their work. A laptop and cell phone are used daily. The desire to help others and yourself succeed. Self-motivated and ability to work independently. Maintains accurate documentation of assigned patient care and treatments provided. Applies knowledge of reimbursement procedures and charge capture. Programs portable infusion pumps for continuous drug delivery and provides instruction to the patient and/or caregiver in the use of the pump at home. Demonstrates knowledge of ambulatory infusion pump use and maintenance, cleaning pumps after use, and proper documentation of pump usage. Maintains work environment Restocks the supply area as needed. Follows OSHA guidelines when handling cytotoxic agents, biohazardous waste and any other material potentially infected with a blood borne pathogen. Maintains all disinfection protocols. Maintains equipment according to quality control standards. Participates in professional development activities and maintains current knowledge of oncology nursing by participating in ongoing educational opportunities. SALARY LEVEL: $40-$60/hour (plus additional float pay) Qualifications Graduate from an accredited program for professional nursing education, BSN preferred. 3+ years nursing experience preferred, preferably at least one year in oncology RN state license and current registration with the State Board of Nursing in practicing state. Current Basic Life Support (BLS) certification. Must maintain continuing education requirements. At the neonatal intensive care unit (NICU) at Children’s Minnesota Minneapolis Campus, our inter-disciplinary team of over 500 professionals is dedicated to the care of babies with life-changing diagnoses. As a key clinical expert and leader within the Trauma Program, the TAPP provides independent patient management, participates in resuscitations, performs advanced procedures, supports quality improvement initiatives, and contributes to trauma education across the organization. Key Responsibilities Clinical Care & Patient Management Develop, implement, and manage care plans in collaboration with Trauma Surgeons and multidisciplinary teams. Independently assess, evaluate, and manage trauma patients across the Emergency Department, PICU, inpatient units, and ambulatory clinics. Respond to all Level I and II trauma activations; perform trauma consults and coordinate admissions. Conduct daily rounds, complete tertiary surveys, and order/interpret diagnostic studies. Perform advanced procedures including airway management, laceration repair, chest tube placement, central/IO/IV access, wound management, and orthopedic interventions such as fracture reductions and splinting. Clear C-spines, document patient care thoroughly, and manage accurate billing. Provide family education, discharge planning, and follow-up calls post-discharge; communicate with referring providers. Quality Improvement / PIPS Participate in PI data collection, review, and follow-up. Attend multidisciplinary PI and trauma team meetings. Identify and escalate PI issues for improvement. Education & Outreach Serve as a resource and educator for nurses, physicians, students, and staff. Provide formal and informal trauma education and participate in workshops, conferences, and outreach activities. Assist with trauma conference planning and guideline development. Policy & Program Support Participate in Trauma Program committees and development of policies, procedures, and clinical guidelines. Support dissemination and evaluation of trauma-related protocols. Professional Practice & Development Maintain required certifications and ongoing trauma CME. Stay current on trauma literature and evidence-based practices. License / Certification Physician Assistant: NCCPA certification and MN Medical Board license required. APRN: MN RN/APRN license; board certification via PNCB or ANCC. All Providers: Current ATLS, BLS, and PALS (ATLS within 3 months of hire). Education Master’s degree required. PA: Graduate of accredited PA program.
