As the Hospice Nurse Practitioner, you will provide consultation in palliative care, symptom management and supportive care to meet the needs of the Hospice patient as requested by the referring physician. : LPN Position Summary Under the general direction of the Wellness Director, primary responsibilities include, but are not limited to providing assistance to the residents in maintaining their highest level of functional independence and psychosocial wellness. This is a full-time role, covering days and some weekends.
