What You’ll Do and Who You Are: You are honest, energetic and approachable with good communication skills and a ready smile. You provide assistance with showering, personal care, medication reminders, going through prescribed range of motion exercises, dementia care, and even hospice care. Meal preparation and homemaking are part of what helps to keep seniors in their home. You don’t have to be a gourmet chef, but you will prepare or assist with preparation of basic meals for your client. You maintain the cleanliness of the home by doing light housekeeping such as vacuuming, dusting, sweeping/mopping floors, doing dishes, changing bed linens and doing laundry. You may run errands, take your client to appointments, and provide companionship activities such as reading, music, puzzles, etc. If you work overnight shifts, they are “awake shifts” in order to provide assistance to our clients. Qualifications: Must be 18+ years of age Must have reliable, consistent means of transportation to work Must have the ability to pass a full employment background check Possess the ability to consistently provide superior customer service to our clients and exhibit exceptional work ethic High school diploma or GED preferred PCA or HHA license is preferred but experience counts! 1 or more years of experience as a Caregiver/Home Health Aide in health or home care also preferred but not necessary Fully vaccinated for COVID-19 as client requested Why Join Our Team? Certified Surgical Technologist | Orthopaedic Surgery Center (OSC)

The Orthopaedic Surgery Center is a collaborative effort between the physicians of Concord Orthopaedics and Concord Hospital. The physicians of Concord Orthopaedics work with anesthesiologists, nurses, and operating room and recovery staff to ensure patient safety and excellence in care. Day surgery centers - like the OSC - are designed for patients who are safe to come in and go home on the same day as their surgery. The procedures we offer at the surgery center are those which can be performed under local, regional or general anesthesia, and are deemed safe for a short post-operative observation period followed by an at-home, same-day recovery. We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities. WOUND NP ROLE AND RESPONSIBILITIES: Provide wound care services for long-term care patients residing in skilled nursing facilities. Prescribe appropriate medications, including controlled substances in accordance with NH Law. Order, review, and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with the primary care physician. WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of New Hampshire. Board Certification. DEA licensure (Optional) Clinical background in adult, family, or geriatric specialties. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting.
