Responsibilities Essential Duties and Responsibilities: Consult with MD/Psychiatrist/LCSW on patients Consult with MD/Psychiatrist on medication management, and/or visit with patient as needed to address BH concerns Provide clinical care (medication management/psych evaluations) for patients to support efforts to achieve care plan goals, in support of ongoing care by MD/Psychiatrist/LCSW, with referral back to ongoing care with clinical team Participate in case conferencing and high-risk meetings with LCSW/MD/Psychiatrist and care teams Approval of treatment plans for patients in BH care Interviews, diagnoses and evaluates patients referred for psychiatric assessment Participates as a team member in conferences concerned with diagnosis and treatment planning Writes reports and documentation as required Consults with other mental health and medical provider staff regarding treatment of patients Prescribes medication as needed and monitors its effectiveness Must be familiar with the process of diagnosing and treating patients with substance use disorder and make appropriate referrals when necessary Examine both children and adult patients and their medical records Order and study diagnostic tests (blood tests, ultrasounds, X-rays etc.) Diagnose health conditions and illnesses Propose treatments for chronic and infectious diseases Prescribe medications Maintain accurate records and schedules Inform patients about their health conditions and prescribed medications Educate patients and their family members on diseases, epidemics and prevention Oversee patients’ overall health Promote good health practices Advance professional knowledge and conduct research 
Description RN/Case Manager MSH Case Management PT Days The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Participates in nursing orientation. 8. Interfaces with other departments in the Medical Center. Qualifications Licensing and Certification Requirements: Education Requirements Bachelors Degree in Nursing; Masters preferred. Experience Requirements Previous experience as in homecare, long term care or utilization review preferred. Discharge Planner or Case Manager preferred. Manager or a minimum of 3-5 years experience as a RN in an acute care setting Certification Requirements: Current NYS RN nursing license. CCM certification preferred. Re-credentialing of CCM status expected. Review medication administration records for completeness at end of shift. Give and receives the nursing report and follow through on any special instructions. When in charge makes periodic checks to ensure that prescribed treatments are being properly administered by LPN’s and CNAs and to evaluate the residents physical and emotional status. Participates in the orientation of new staff residents and families to the unit. Carries out restorative and rehabilitative programs including teaching resident to wash/feed/dress self as appropriate. Ensures that the facility policy on the use of restraints is followed and that documentation is timely and accurate. Ensure that the unit is kept clean free from clutter and that the supply room door is kept locked; request the supervisor’s intervention as necessary to achieve this. Follows infection control standards and ensures that assigned personnel do likewise. Keep abreast of new nursing policies and procedures. Keep nurses notes, plan of care and all other documentation pertaining to your assigned resident timely accurate and complete. Carries out end of shift responsibility e.g. supplies are available for next shift’. Unit is clean and tidy; all documentation is complete and accurate. Responsibilities The primary purpose of the Operating Room Technician I position is to be under the direct supervision of the professional nurse. The OR Technician I performs various duties to assist with surgical procedures and related operating room activities. The OR Technician I delivers care as reflected in the Association of Surgical Technologists’ Recommended Standards of Practice in a manner that is cost effective without compromising quality of care. The OR Technician I possesses knowledge of the theory and application of sterile and aseptic technique and combines the knowledge of human anatomy, surgical procedures, and implementation tolls and technologist to facilitate a physician’s performance of invasive, therapeutic, and diagnostic procedures. The OR Technician I demonstrates competency to scrub for minor/major surgical and endoscopic surgical procedures. An Operating Room Technician I is a certified technician. The OR Technician I participates in departmental quality assessment and improvement activities, meets hospital continuing education and certification requirements, participates in student teaching activities, upholds Patient Care Services philosophy and objectives, demonstrates knowledge of Association of Surgical Technologists’ Recommended Standards of Practice, AORN’s Age Specific Competency Statements and demonstrates professional growth and development. Measurement of performance behavior is “as observed by Nurse Manager and/or Educational Specialist according to OR policy and procedure” unlessotherwise indicated. Qualifications Graduation from an accredited Surgical Technician Program. Candidates must be eligible to sit for the LCC-ST certifying examination. One (1) year of related experience preferred. Certification as an Operating Room Technician (LCC-ST) plus one (1) year of related experience preferred. Constant walking, bending, stooping and standing for long periods of time. Pushing and pulling of heavy objects such as gurneys, patient beds and OR beds. Must maintain a neat, clean, professional appearance. Must have understanding, patience, and tact in dealing with physicians and fellow workers. Must pay attention to detail. Must have excellent hand-eye coordination. Must be selfmotivated and work in an efficient and effective manner. Must work in air-conditioned environment with temperature of 65-73 degrees and humidity of 45-55%. When assigned to pediatric cases, temperature may be up to 85-90 degrees F. Must wear hospital provided scrub attire while on duty. Potential for cuts, bruises, muscle strains and exposure to contagious disease via and body fluids. Potential for exposure to anesthesia waste gases, ethylene oxide, harsh disinfectants, sterilizing agents, high steam pressure agents and radiation. Willingness to work with employees at various levels of skill, knowledge and abilities.

How you make a difference As a Registered Nurse (RN), your primary responsibility is to provide patient care using the nursing process of assessment, planning, implementation, and evaluation. You will work under the supervision of Charge Nurse(s) to provide patient education and activities in accordance with your competencies and education. Count controlled substances and implement clinical and technical aspects of care in accordance with established policies, procedures, and protocols. Document nursing encounters using the SOAP form of charting as required by policy and attend mandatory staff meetings and training. Communicate information to nursing staff, physician, health care unit supervisory personnel or other staff as necessary. Overview About the Role The Advanced Practice Nurse (APN) is a skilled health provider who through a written collaborative practice agreement with a physician is credentialed through the Medical Staff Office. As a member of the Medical Staff the APN demonstrates standards of professional practice that includes ethical decision making, job activities related to quality assessment/improvement, self-evaluation, leadership, and research. The APN is self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency. The APN in the Surgery Practices utilizes critical judgment in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacologic and non-pharmacologic treatments in the direct management of patients diagnosed with a variety of surgical conditions/diseases. The APN practice will promote wellness through conducting research, providing education and works autonomously as well as in interdisciplinary teams. Responsibilities What You’ll Do Collects comprehensive patient health data/history including past and present medical, surgical, family, and social history. Diagnostic tests and procedures relevant to the patient’s status are collected, interpreted, and documented at the Electronic Medical Record (EMR). Completes the medication reconciliation process by documenting the current list of medications (including over the counter and nutritional supplements) taken by patient at each visit. Performs and documents an appropriate comprehensive physical assessment of patients which will include a review of systems, evaluation of manifestations and toxicities related to medical conditions and anticoagulation management. In collaboration with the physician, critically analyzes the assessment data/diagnostic tests and determines a diagnosis. In addition, develops a comprehensive plan of care that includes interventions of treatments to attain outcomes. Evaluates patient progress in attaining expected treatment outcomes. Monitors for signs and symptoms related to disease progression/recurrence. Appropriately manages medical and anticoagulation management emergencies according to protocol/policy. Prescribes and/or orders medications. Obtains appropriate consents for surgery and procedures. Ensures completion of all needed pre-operative tests, forms, and documents. Documents assessments, prescribing, plan, modifications to plan, and evaluation of care in the EMR. Qualifications What You’ll Bring Required: Completion of an educational program which is designed and conducted to prepare graduates to practice as an NP/APN with a master’s degree in nursing and eligible for certification by a national certifying body. A current Registered/Professional Nurse license and registration in the State of New Jersey is required. Evidence of excellent interpersonal skills, effective communication skills, and creative problem solving are essential. A minimum of two (2) years of clinical nursing experience and current BLS certification are required. To perform this job successfully, an individual must be able to perform in an expanded nursing role by providing specialty care to General Surgery patients in collaboration with the attending staff. Responsibilities The primary purpose of the Advanced Practice Nurse (APN) position is to provide high quality patient care. One (1) year of experience as an Advanced Practice Nurse in Emergency, Critical Care, Adult or Family preferred. Demonstrates effective leadership competencies to be carried out in the day-to-day activities of the department. BLS (CPR) and ACLS required. Computer literacy required.
