Join UChicago Medicine , as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) within the Heart Failure department. This is an Outpatient position. As an Advanced Practice Provider , The APP’s primary responsibilities include but are not limited to, managing independent clinic templates and working alongside the Cardiology Providers in clinic. The patient population includes complex general cardiology and post hospital discharge patients. Administrative duties including, triaging and responding to Epic inbaskets; responding to refill requests, peer-to-peer provider reviews, diagnostic scheduling and result communication. APP must be flexible with other duties as assigned. Candidate must be accustomed to diagnosing and managing a full range of cardiology conditions. Essential Job Functions Provide daily management of complex Cardiology patients, familiar with common cardiology diagnostics - EKG’s, holter/zio results, stress testing, and echocardiograms Comfortable identifying and referring patients appropriately to specialized areas of Electrophysiology, Interventional Cardiology, Advanced Heart Failure. Responsibilities ESSENTIAL DUTIES AND RESPONSIBILITIES: - Assesses, plans, implements, and evaluates care for patients. - Initiates and documents appropriate interventions; evaluates and records patient/family response and prescribes therapies as indicated. - Performs procedures with minimal supervision. - Assesses patient/family needs and provides education. - Promotes wellness and recommends community resources to meet patient and family needs. - Often incorporates evidence-based research into clinical practice. - Solid knowledge of reimbursement procedures and documentation requirements. - Completes patient charge capture and medical record documentation in a timely fashion. - Likely to provide case management for patients and their families. - Sometimes serves as a resource for nursing staff by providing information being studied in clinical trials. - May provide palliative care for the patient and their families as needed. - May promote/provide staff education - Responsibilities may include rounding at affiliated hospitals, weekend or after-hour calls - May mentor new advanced practice providers - May precept APP students - Other duties as assigned Qualifications MINIMUM QUALIFICATIONS: Masters of Science in Nursing (MSN) from an accredited nursing school and successful completion of a Nurse Practitioner program. This position typically requires a minimum of 3 years of experience as a practicing Nurse Practitioner. Previous oncology experience strongly preferred. RN state licensure and current registration with the State Board of Nursing in practicing state required. Current state licensure as a Nurse Practitioner required. Current basic life support (BLS)/CPR certification required.
