JOB DESCRIPTION Job Summary Provides support for care management/care coordination long-term services and supports (LTSS)-specific activities. Collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum for members with high-need potential. This position will be assisting members with Medicaid and Medicare benefits. This position may require home visits if needed for care plan review and signature. This position may involve more than one clinic visit per day, depending on the needs of the facility. Requirements: Proven ability to perform all tasks and duties of a Home Health Aide. Strong command of the English language (spoken, written, and reading comprehension). Expertise in providing age-appropriate care to residents. Able to make independent decisions when needed and work well with staff, residents, families, and external partners.
