Overview Medical/Surgical (ACE) 3-North: Joining this forty-two-bed unit can be a very rewarding step to initially pursue, or to further expand on your already established career in med/surg nursing. With a patient population encompassing mostly medical patients, with roughly 65-70% of the patients being geriatric, the educational and professional opportunities abound. Caring for the elderly population has the potential to bring both personal and professional satisfaction and growth, as this subset of patients have a lot to offer from every facet of nursing. CentraState Medical Center currently has an employment opportunity available for a Clinical Leader in the Medical/Surgical (ACE) 3-North unit. The Clinical Leader plays a pivotal role in coordinating culturally sensitive care from pre-admission through post-discharge. This includes managing resources to achieve optimal clinical and financial outcomes, supporting quality service delivery, and guiding staff in evidence-based nursing practice. The role also assists the Nurse Manager in overseeing unit operations on a 24-hour basis and serves as a clinical resource and educator to team members. Care is delivered through a multidisciplinary, patient-focused model aligned with CentraState’s organizational goals and standards. Responsibilities Assists the Department Head in the continuing effort made by the department to improve the quality of patient care and services. Provides considerate respectful care focused upon the patients' individual needs. Affirms the patient's right to make decisions regarding his/her medical care, including the decision to discontinue treatment, to the extent permitted by law. Demonstrates the ability to obtain information and interpret information in terms of the needs of the age of the patients cared for. Demonstrates an understanding of the range of care needed based on the age of the patients cared for. Successfully completes life-span education as appropriate. Successfully completes life-span education as appropriate. Obtains a complete nursing history and baseline assessment that is systematic, consistent, and continuous. This assessment is communicated effectively and documented. Appropriately analyzes health status data to determine nursing diagnosis and needs for appropriate nursing care, medical intervention or services of other clinical professionals. Identifies patient problems and formulates nursing diagnosis according to physical, psychological, spiritual/cultural and educational needs in conjunction with a multi disciplinary approach. Consistently includes patient and family in the development of an individualized plan of care. Administers medications as needed following CSHS policies and manufacturer's instructions. Takes immediate and appropriate actions in the event of a medication variance occurrence. Attends and participates in the multi-disciplinary discharge planning meetings and identifies the need for further services following discharge. Provides appropriate education based upon teaching plan. Conducts unit based education programs on a routine basis in collaboration with professional development. Meets standards for documentation regarding patient education. Documents results of patient assessment, nursing diagnosis, care plan nursing interventions and evaluation and patient progress in Medical Records, Nursing Standards of Care/Practice for the specific patient type. Recognizes changes in patient condition and/or emergencies situations and takes immediate and appropriate actions. Delegates components of care to non-professional caregivers as appropriate and evalutates outcomes. Makes decisions that demonstrate knowledge of nursing practice. Utilizes sound nursing judgment in clinical and patient care management situations. Demonstrates critical thinking skills in the care of patients and in the coordination of care to assure the achievement of maximum outcome. Develops new clinical pathways, monitors patient progress on current clinical pathways, reviews variances with Patient Care Reviewers, reviews variances from Quality Management with the Director/Nurse Manager and staff and reports to Nursing Quality Council. Follows the clinical progress of all patients. Participates in the development of unit protocols as needed. Conducts unit conferences to discuss quality issues and quality improvement strategies. Quality Referrals - unit based, follows up on nursing referrals, investigates problem, develops action plan, reviews with Nurse Manager, responds to Quality Management in writing, implements action plan. Follows up on patient complaints or concerns and keeps Director/Nurse Manager informed. Provides or coordinates at a minimum, 1 formal in-service per year to address unit specific needs. Monitors appropriate usage of speciality and Zonaire beds. Consistently adheres to hospital policy regarding the insertion, maintenance and removal of foley catheters, if applicable. Able to verbalize and demonstrate proper rationale and techniques surrounding all aspects of indwelling urinary catheter care as per policy # 4668. Fulfills the requirements for competencies and completion of annual mandatories surrounding indwelling urinary catheters. Takes a patient assignment when necessary. Qualifications Bachelor's Degree in Nursing required. Master's Degree in Nursing required or must obtain within three years from date of hire (or internal transfer) as a condition of employment. Two years of Nursing experience required. Supervisory experience preferred. NJ RN License required. CPR Certification required. ACLS Certification to be achieved within first year. Successful completion of Basic Dysrhythmia course to be achieved within first year. CSMC IV Certification to be achieved within first year. Certification in specialty preferred. Duties & Responsibilities Purpose of Position : Under supervision, provides professional nursing care to patients/clients within an assigned unit. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and provision of care supportive to and restorative of life and well-being. Examples Of Typical Tasks Identifies and discriminates between physical and psycho-social signs and symptoms. Selects and performs those therapeutic measures which are essential for effective execution of the nursing regimen for assigned patients/clients. Develops, implements and continuously evaluates the nursing regimen for assigned patients/clients. Records and maintains nursing care plans and progress notes on patients/clients to ensure continuity of care. Interprets and reports responses of patients/clients to appropriate members of the health team. Participates with interdisciplinary health team members to plan a comprehensive patient care program. Determines and assigns those components of the nursing regimen which may be carried out by allied nursing personnel. Special Note For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit.
