All affiliated Home care aides employed by Nurse Next Door to provide in-home care services will be 18 years old (or if under 18 years of age be a high school graduate or enrolled in a vocational education program) and have sufficient communication and language skills to enable them to perform their duties and interact effectively with clients and other agency staff. Home care aides must meet all registration, criminal background checks and training requirements as specified by the State of California Department of Social Services Home Care Services Bureau and Nurse Next Door policies. The Hospice Team Nurse completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. POSITION QUALIFICATIONS 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. 7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. 
Job Description Under g eneral supervision of the Nurse Supervisor and Administrative Director of Ophthalmology, guided by the policies and procedures of the Ambulatory Services, and in collaboration with the medical director and physician staff, the Clinical Nurse II (CN II) performs established nursing procedures using clinical knowledge to care for adult and pediatric patients. Job Summary: Selects and administers intravenous, spinal and other anesthetics prior to and during surgical or medical procedures, as a member of the anesthesia care team and in accordance with Kaiser Permanente policies and procedures. Monitors patient reactions and vital signs during and after anesthesia and takes remedial action when warranted. Submits postoperative report on patient condition, and maintains records of anesthetics administered. Evaluates patients to identify apparent difficulties with airway control, etc. Ensures that all pre-anesthesia evaluations are documented thoroughly according to policies and standards. Explains tests, procedures and disease processes to patients and their family as indicated; explains the major anesthetic risks to patients as requested and documents that the risks are explained to patients. Develops the care regimen of assigned patients; reviews the care regimen with the supervising physician and gains concurrence; implements the anesthesia plan; starts IV solution, administers selected anesthetic, choice of anesthetic consistent with patient safety, surgical need, possible post operative complications, and patients history and conditions. and evaluates and manages the patient, according to departmental policy. Participates with post anesthesia care unit personnel in a patient safety oriented interface when patients are transferred from the surgery suite.
