Provide input to the nurses for development of the plan of care. Provide adequate hydration to residents at regular intervals each day according to specific resident needs. Prepare residents for each meal and transport them to dining room (unless a bed patient) making sure that food tray is accessible, self-help devices are available (if needed), and assistance is provided to those residents who cannot feed themselves. Keep an accurate record of food intake and output, as directed. Be alert for proper ventilation, temperature, light and noise control. Take TPR (temperature, pulse, & rate), blood pressure, weighing of residents, etc., as directed. Promptly answer call lights and assist with other resident needs. Advocates for health and accesses hospital and specialty clinic services. Provides supervision to the Licensed Practical Nurse(s) on shift. POSITION DUTIES: Assesses each person receiving services to determine and identify needs, issues, resources, and health care outcomes. Identifies individual’s development strengths and health care needs. Obtains emergency medical care when necessary. Develops and updates health care plan to address assessed needs. Provides direct medical/nursing services to assigned individuals as presented. Oversees the implementation of the health care plan by training persons receiving services, family, and/or caregivers. Trains and oversees staff in carrying out treatment plans relating to health needs on an ongoing basis. Evaluates effectiveness of plan in meeting outcomes.
