Complete and ensure LPN and C.N.A. documentation of care and services rendered according to facility policies and procedures. Complete reports such as the 24-hour change in condition report, Incident and Accident reports, etc. according to the facility policy and procedure and as assigned by the Director of Nursing Services. Provide immediate response to situations of resident risk such as allegations of abuse, neglect or misappropriation of property. Report incidents timely and to appropriate persons per facility policy. Ensure timely and accurate medication and treatment administration. Communicate to physician and documents changes in resident condition utilizing both objective and subjective nursing assessment. Document and implement changes in nursing and medical plans of care. According to facility policy and procedure, assure that all members of the interdisciplinary team are aware of and understand changes in resident nursing and/or medical plan of care. Position Requirements Associate's Degree from credentialed school of nursing. Job Summary We are seeking a part-time, 25-hour Medical Assistant to join our Express Care team! Express Care, formally known as Urgent Care, offers timely treatment for non-emergency conditions. We care for adults and children ages 3 and up, addressing a wide range of issues such as respiratory illnesses, infections, minor injuries, rashes, and stomach problems. Because patients often visit when they’re feeling unwell, our team prioritizes compassion and emotional support alongside quality medical care. The schedule consists of working 2, 12.5-hour shifts per week (8:30am to 9:00pm), including every other weekend (both Saturday and Sunday). Since Express Care is a 7-day-per-week operation that delivers care during the day, evening, weekends, and holidays, the schedule for this role will require 2 on-call shifts per 4-week scheduling period, weekend, holiday coverage and the flexibility to work overtime as needed. The primary location for this role is 1285 Beacon Street Brookline. We do our best to keep all employees at their assigned centers; however, for on-call shifts, employees must be able to commute to any of our locations, including Brookline, Newton, Lynn, Natick, Westwood, and Woburn. You may also be asked to report to any of these centers during orientation (for training/precepting purposes) or during times of staffing shortages or other extenuating circumstances (for cross-coverage purposes). The Licensed Practical Nurse in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home. Primary Responsibilities: Provides high quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team, following all applicable infection control procedures Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency's electronic visit verification program (as applicable) Actively participates in the agency's care coordination process including timely reporting patients' needs and changes in condition, attending patient case conference, communicating effectively to the patient, family, physician, other clinicians, agency management staff, and case coordinator, community case managers (as applicable) Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Current and unrestricted LPN licensure in state of practice Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation Preferred Qualifications: Current CPR certification or ability to complete within 90 days of hire Home care experience Ability to manage multiple tasks simultaneously Able to work independently Good communication, writing, and organizational skills Ability to work flexible hours as required to meet identified client needs Pay Range $54,324 - $81,487 annual total cash target pay $26.12 - $39.18 hourly rate $31.34 - $47.01 per visit point Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of 'direct' and 'indirect' patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable.
