No evenings, weekends or holidays. Floating is required as this role will be travelling to various locations and facilities within Northeast Ohio. A caregiver who excels in this role will: Assist with performing screenings and treatments by gathering and updating information related to health maintenance, identifying medication changes, updating allergy information, collecting specimens, performing tests and documenting patient care. Monitor and communicate changes in patient condition. Maintain equipment, medical supplies and examination areas. Perform administrative/clerical duties, such as filing, reception, scheduling, data entry and patient registration. Minimum qualifications for the ideal future caregiver include: Graduate from an approved Medical Assisting Program (As designated by the Enterprise MA Steering Committee) Completion of a clinical externship (Extensive clinical experience may be considered in lieu of formal clinical externship) Successful completion of Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross (within new hire period) Current demonstrated clinical proficiency Military training as a Hospital Corpsman (HM), Combat Medic (68W) or Medical Service Technician (4N0X1) will be considered Preferred qualifications for the ideal future caregiver include: Medical Assisting Certification Successful completion of Cleveland Clinic Externship Physical Requirements: Manual dexterity to operate office equipment. The Licensed Practical Nurse provides nursing care at a DCI in-center hemodialysis clinic as part of a patient care team. What You'll Do as a Hospice RN / RN Case Manager: Be the primary point of contact for patients and families, guiding them through every step of their hospice journey. Help patients and families understand and navigate topics such as: Medication administration Hospice philosophy and services Symptom and pain management End-of-life processes and expectations About You Qualifications – What You’ll Bring: Active RN license in the state of employment (or eligible to obtain). 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields. A genuine hospice heart: compassionate, empathetic, and patient-centered. Comfortable providing care in diverse settings, including private homes and facilities. Strong communication and critical thinking skills in emotionally sensitive situations. Valid driver’s license, auto insurance, and reliable transportation. CPR certification required.
