Overview Department: Emergency Unit Schedule: Nights, 12-Hour Shift Hospital: Resurrection Medical Center Location: Chicago, IL 60631 Responsibilities The Registered Nurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Provides direct patient care within the scope of practice. The Registered Nurse also directs and guides patient/family teaching and activities of other nursing personnel while maintaining standards of professional nursing care in Telemetry. The Registered Nurse is directly responsible and accountable for the care given to his/her patients in the Emergency Department. They communicate with the physician about the changes in the patient’s clinical condition including results of the diagnostic studies and symptomatology. Is capable of responding quickly and accurately to changes in the condition and/or response to treatment. Is knowledgeable about EMTALA regulations and reporting of child/adult abuse to appropriate agencies and Hospital Social Worker. Maintains patient privacy and confidentiality. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current and valid state RN License.2. Current BLS certificate upon hire and maintain current.3. Current ALS certificate 30 days upon hire and maintain current. 4. Current PALS certificate and/or ENPC 60 days upon hire and maintain current. 5. Certified Emergency Nurse (CEN) preferred.6. Bachelor of Science in Nursing (BSN) preferred. Facility Specific Requirements (facility may require items listed below): 1. Attends and actively participates in resident care plan conference, and Resident Council and/or Family Council when invited. Reports all concerns, complaints made by the customer or their family to the Administrator. Makes rounds to assure customers are safe and comfortable. Maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department. Participates in In-service Education Programs. Assists in implementing and maintaining an effective orientation program that orients the new Ladies and Gentlemen of Alden to the facility, his/her department the wound program, and its policies and procedures. Performs personal services for customer’s (i.e., reading, writing letters, etc.) as time allows. Encourages customers to help themselves and promotes self-care in aiding rehabilitation. Assists with care of dying and expired customers. Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident's eating habits to the Staff Nurse. Add key accountabilities here. Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule. Assesses assigned case load of home health care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations. Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care. Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. What is Required? Current, unrestricted state RN licensee 1+ year experience in a clinical care setting as a nurse Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Able and willing to travel within branch/office coverage area. Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations Must be able and willing to travel 50% Associates Degree or Bachelor’s Degree in Nursing is preferred Previous Home Health or Hospice Experience is preferred Previous experience with HomeCare HomeBase is preferred You will report to the Clinical Manager or Clinical Supervisor The base salary for this position is $55.00 per point, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
