Key Responsibilities MDS & Assessment Management: Coordinate, schedule, and complete all required MDS assessments (OBRA and PPS) accurately and in strict compliance with federal and state timelines. Care Planning: Facilitate interdisciplinary care plan meetings, working closely with nursing, therapy, social services, and dining teams to establish individualized care plans. PDPM & Reimbursement Oversight: Monitor and optimize Patient-Driven Payment Model (PDPM) coding, ICD-10 diagnosis coding, and documentation to ensure accurate reflection of care delivered and proper clinical reimbursement. Triple Check & Auditing: Participate in weekly Triple Check meetings prior to billing submission; perform routine audits of clinical documentation to support MDS coding. Staff Education: Educate floor nurses and interdisciplinary team members on documentation standards, MDS coding requirements, and care plan compliance. Qualifications Licensure: Active, unencumbered Registered Nurse (RN) license in the State of Illinois. Experience: Minimum 2+ years of experience as an MDS Coordinator in a Skilled Nursing Facility (SNF) or long-term care setting. Certification: RAC-CT (Resident Assessment Coordinator-Certified) preferred. Regulatory Knowledge: Deep understanding of CMS RAI guidelines, Medicare Part A/B, Medicaid, PDPM, and IDPH long-term care regulations. Technical Skills: Proficient with PointClickCare (PCC) or similar Electronic Health Record (EHR) systems. PICU nurses participate in an inter-professional approach to the quality treatment of patients and their families with medically complex diseases, organ transplantation, multiple traumas, various respiratory illnesses, and many other pediatric critical conditions. The latest interventions and treatments are available in the PICU, including extracorporeal membrane oxygenation (ECMO), Continuous Renal Replacement Therapy (CRRT), specialty ventilation techniques, and transplant services. The PICU provides family-centered care through bringing family members into care plan meetings and daily rounds which are led by PICU nurses. Additionally, PICU nurses care for patients in the robust Pulmonary Habilitation Program which supports families as they train to care for their child who will be discharged home on a ventilator. MDS Coordinator Position: MDS Coordinator Department: Clinical / MDS Employment Type: Full-Time Salary: Based on experience and qualifications Employment Type: Full-Time Job Summary The MDS Coordinator is responsible for coordinating and overseeing the facility’s Minimum Data Set (MDS) assessment and Resident Assessment Instrument (RAI) process to ensure accurate, timely, and compliant assessments and appropriate clinical reimbursement. This position works closely with nursing, therapy, restorative, medical, and administrative teams to ensure compliance with federal and state regulations and facility policies. The MDS Coordinator reviews clinical documentation, monitors Medicare and managed care residents, completes and coordinates MDS assessments, identifies documentation and compliance concerns, and provides education to facility staff regarding MDS, Medicare, RAI, and reimbursement requirements.
