DaVita Kidney Care is bringing our industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line. Ideal teammates will have experience in dialysis, get energized by a start-up type environment and improving the lives of patients, and have a track record of finding ways to Get Stuff Done (GSD). Teammates must be comfortable working autonomously and have demonstrated the ability to collaborate with cross-functional resources across the organization. What you can expect: In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. DSD RNs will troubleshoot, and make sound decisions for our patients related to their dialysis treatment and will occasionally partner alongside the SNF staff to coordinate care. Primary work schedule will be 3-4 days a week with travel to other SNF clinics within your region (depending on business and patient needs). Alternating Saturdays will also be required. As a DSD Services RN, you will be provided the opportunity to conduct impactful and meaningful education to patients and their families in the SNF setting. Direct Patient Care: The RN’s primary responsibility is to independently demonstrate best practice techniques and deliver optimal dialysis care. The role includes supervising Dialysis Technician who are tasked with delivering tandem care to meet the capacity and efficiency goals of the program. Complexity - DSD Dialysis care is a complex but fulfilling service line. Meeting and/or exceeding clinical outcomes are the goals and multitasking is an expectation to meet the needs of the service, the patients and our SNF partners. Autonomy and Critical Thinking. On an Average Day You Will: (includes but not limited to) Perform personal care needs after given a patient's assignment and attend to his/her requests promptly; if unable to perform a certain task, report to the Case Manager immediately Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality Meet safety needs of patient and use equipment safely and properly (foot stools, side rails, O2 etc.) Provide personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned Assist in dressing and undressing patients, as assigned Plan and prepare nutritious meals, including shopping, as assigned Assist in feeding the patient, as assigned Take and record oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence Assess and report on patient's condition and significant changes to the Case Manager; also, being aware of the caregiver or other individuals living with the patient and interpersonal issues Assist in ambulation and exercise as instructed by the hospice nurse or therapist Provide normal range of motion and transfers/positioning, as directed Assist with self-administered medications as allowed by state regulations, such as “reminders” Offer and assist with bedpan and urinal aid, as assigned Aid with light laundry needs if necessary Perform range of motion and other simple procedures as an extensional therapy service, as ordered with appropriate completed/demonstrated skills competency Provide respite for patient's family/caregiver when on-site, as appropriate Maintain patient's living area is clean and orderly, as assigned Adhere to the organization's documentation and care procedures and standards of personal and professional conduct Participate in quality assessment performance improvement teams and activities Respect patient and family/caregiver environment and patient's personal needs All other duties as assigned Requirements: Must be a Certified Nursing Assistant ( applicable for all States, additional requirements for California residents, see below).
