Position Summary We are seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our hospice team. The Admissions RN is responsible for evaluating patients for hospice eligibility, completing admission assessments, coordinating the start of care, and educating patients and families about hospice services. Develop and initiate individualized plans of care. Educate patients and families on hospice philosophy, services, medications, and expectations. Coordinate admissions with referral sources, physicians, hospitals, nursing facilities, and case managers. Obtain informed consents and ensure all regulatory requirements are met. Collaborate with clinical staff to facilitate seamless transitions of care. Maintain compliance with Medicare, state, and agency regulations.
