Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with MSMC Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice. Responsibilities 1. Oversees clinical operations within the program, using data and fact-based problem solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. 2. Recruits, orients, trains, coaches, counsels, mentors, disciplines, and evaluates staff in accordance with all internal policies and procedures. Develops, maintains and reconciles departmental budget with financial responsibility and oversight for department or division. 4. Oversees the collection, compilation, and analysis of clinical program activity data to evaluate clinical performance and competence. 10. Provides technical assistance, resource services, consultation and education to clinical specialty service relative to orientation, ongoing assessment of competency and performance improvement activities. 11. Serves as a clinical role model and resource to the nursing staff. 12. Prepares reports and analysis associated with orientation of new hires, ongoing competency and evaluation of staff performance and identification of related outcome measures. 13. Provides timely status reports of results of assignments including identification of patterns and trends that require actions. 14. Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations. 15. Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits. 16. Participates in daily activities that reflect budgetary constraints and incorporates financial principles into program development and allocation of resources. 17. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies. 18. Participates in the performance appraisal process of designated Nursing staff. Qualifications Master's Degree in Nursing is required. NP Certification in specialty area preferred. 5 years directly related experience (some supervisory preferred) New York State License with current registration as a registered professional nurse. Description RN/Case Manager MSH Case Management FT Weekdays/Weekends/Holidays The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Required Qualifications: Active and unrestricted registered nurse with current license in state of NY Two (2)+ years of experience working as an RN in a Critical Care setting that contributes to proficiency in infusion therapy (such as medical surgical unit, ED, ICU, post-anesthesia care, or similar setting) Experience with lab draws, IV infusions, and reconstitution of medications Strong experience with working with a variety of pumps (CAAD, curling, etc) & troubleshooting issues that may arise Possess a valid and current driver’s license and auto insurance/registration Ability to handle specialty cases like IGG, IVIG, Solumedrol, etc. Have and maintain current CPR / BLS certification Knowledge of HIPAA patient privacy and data security requirements; OSHA employee protection regulations Hold current Malpractice Insurance Have experience with Microsoft Office to document and track patient care activity About Us: Founded in 2008, UniversaCare is a fully licensed and growing Home Care Agency serving the NY Metropolitan area and is continually expanding throughout the Northeast.
