Inform reception desk of provider’s schedule status and assist in coordination of urgent and non-urgent provider schedule changes to ensure patient and provider needs are met. Perform patient correspondence when requested and assist patients in completing forms when appropriate. Prepare and sterilize instruments and equipment according to procedure and OSHA standards. Complete scheduled cleaning requirements for designated work areas. Document completion of work to comply with infection control and OSHA standards Implement other infection control procedures as needed and adhere to protective clothing requirements. Ensure clinic supplies are adequately stocked and well organized. Process and deliver department inventory requests in a timely manner. Routinely check expiration dates and remove or replace outdated supplies. Maintain prescription sample supply cabinet as per clinic protocol Ensure all equipment is properly maintained and adequate records are kept. Ensure inventory of instruments and equipment is current. Ensure instruments and equipment are calibrated as needed and maintained in proper working order. The Medical Assistant and LPN position requires the ability to sit or stand for prolonged periods of time. Must possess speech, hearing, and sight skills to perform the functions of the position. Manual dexterity required. Must be able to speak, read and write English proficiently. Proficiency in other languages may be required. Some positions may have lifting requirements. Job Overview Ridges Hospital is looking for a passionate and hardworking Nursing Assistant to join our Float Pool unit! Located in Burnsville, just 20 minutes south of Minneapolis, MN, near the 35 E and 35 W conjunctions. This position is a .9 FTE, working 72 hours per 2 week pay period. This position works 12-hour shifts, every third weekend rotation (Fri,Sat,Sun), and on the night shift (7pm-7:30am). The Nursing Assistant is responsible for assisting the professional nurse/caregiver in the delivery of patient care, transporting patient and/or equipment, setting up and maintaining patient rooms, and the reordering and stocking of supplies. The Nursing Assistant does not work autonomously but works under the direction of a professional nurse. The Nursing Assistant may be given a patient care assignment based on unit needs and the discretion of the manager/charge nurse. Provides basic patient care by performing routine duties to include: Personal grooming, bathing, toileting, Feeding, Skin care needs (clean and sterile dressing changes), applying ointments/creams, Oral hygiene, Specimen collection/testing, Position/ambulating, Changing linens, Promptly answers call lights. Assists the nurse in monitoring patient condition. Accurately takes and records weights, input/output, blood pressure, temperature, pulse rates, respiration, oxygen saturation. Places patient on/off monitors or equipment. Reports unusual findings to the RN in a timely manner. Assists with admissions, transfers, and discharge of patients. The on-call registered nurse organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse directs the immediate services necessary to meet the needs of individual and families within their homes and communities. This position would help cover call for nights and weekends. This position is a Full-Time position working 7 days on 7 days off. The working hours for the 7 days on are: Monday-Thursday 5:00pm-8:30am; Friday 5:00pm - Monday 8:30am. Responsibilities: Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs for after-hours calls. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. After hours on-call responsibilities included remaining available for all calls related to patient care and making all appropriate visits as necessary. Remaining with patient and or family through a crisis period until resolved and or relived. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. Provides and maintains a safe environment for the patient. Assists the patient and family/caregiver and other team members in providing continuity of care. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. Attends interdisciplinary group meetings. Ensures that arrangements for equipment and other necessary items and services are available. Supervises ancillary personnel and delegates responsibilities when required. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. Fulfills the obligation of requested and/or accepted case assignments. Actively participates in quality assessment performance improvement teams and activities.
