Description Under the supervision of a licensed physician responsible for development, implementation, coordination and evaluation of clinical programs and services for designated patient populations. Provides evaluation and direct patient care that promotes achievement/maintenance of optimal health. Responsibilities Provides general patient care under the direction of or in consultation with the Chief of the Division. Evaluates patient’s physical, emotional, and psychosocial status, and makes recommendations for treatment or referral as appropriate. Prepare and help Attending Physician performing FNA of thyroid nodule. Give appointment to patient for FNA and coordinate the timing with the Ultrasound Department. Participates in meetings with the physicians and other team members to discuss progress and/or management of specific patients. Performs other duties pursuant to clinical responsibility, such as arranging follow-up for patients, obtaining informed consent from patient, recalling patients for further evaluation and treatment, escorting patients, when necessary, to other areas of the hospital, etc. Informs patients of necessary post-discharge instructions as required. Makes appropriate entries in patients’ charts. Demonstrates professional involvement by attending and participating in seminars, workshops and courses as presenter or author; keeping abreast of current literature and trends in practice; considers ethical issues in professional nursing practice. Attend and help the Clinical Diabetes Educators in support group sessions for Diabetes patients. Work with Clinical Research Coordinator on research protocols, entering data and follows of patients participating in clinical studies. Qualifications -Job requires completion of NYS-approved program for training of registered professional nurses and an accredited program in Nurse Practitioner in order to gain knowledge of anatomy, physiology and clinical patient care. Master’s Degree and certification in specialty area are preferred. -Two years progressive job related experience specific to area of responsibility. -Registration required with New York State as Registered Professional Nurse and Nurse Practitioner. -Certification by ANCC in specialty area. -High level of analytical ability required to effectively and expeditiously diagnose, monitor and deliver care to patients under the direction of physician. -Interpersonal skills sufficient in order to effectively interact with patients to gain trust and cooperation. This provider will serve as a primary clinician for medication management, caring for a diverse patient population with complex chronic pain conditions and multifaceted medical histories. The ideal candidate will practice evidence-based pain management, prioritize patient safety, and work collaboratively within a multidisciplinary team to deliver comprehensive, patient-centered care. Essential Responsibilities ● Serve as the primary provider for medication management of patients with acute and chronic pain conditions. ● Evaluate new patients and develop individualized, evidence-based treatment plans in collaboration with the interdisciplinary pain management team. ● Manage opioid and non-opioid analgesic therapies while adhering to federal, state, and institutional prescribing guidelines. ● Maintain and update annual controlled substance/pain management agreements. ● Perform routine monthly monitoring of the Prescription Monitoring Program (PMP/PDMP) in accordance with clinic policies. ● Provide ongoing opioid risk assessment and risk stratification utilizing validated screening tools and clinical judgment. ● Order, interpret, and maintain appropriate urine drug testing and other monitoring measures to promote safe prescribing practices. ● Monitor patients for medication effectiveness, adverse effects, compliance, and opportunities to optimize therapy. ● Perform low-risk office-based procedures, including but not limited to trigger point injections and bursa injections, as credentialed. ● Provide follow-up care for patients undergoing interventional pain procedures performed by interventional pain medicine physicians. ● Educate patients and families regarding pain conditions, treatment options, medication safety, and realistic treatment expectations. ● Document patient encounters accurately and thoroughly within the electronic medical record. ● Participate in quality improvement initiatives and support the continued growth and development of the Pain Medicine program. ● Attend and actively participate in monthly provider meetings, multidisciplinary case conferences, and morbidity and mortality discussions. ● Support departmental goals and organizational initiatives established by Broadlawns Medical Center. ● Foster a positive, respectful, and collaborative work environment through effective communication with physicians, advanced practice providers, nursing staff, medical assistants, and ancillary departments. ● Demonstrates flexibility to adapt to changes in workload, schedule, department function and procedures. ● Participates actively in creating a positive and harmonious work environment.
