HOW YOU'LL WORK: You'll provide skilled nursing care in a patient's home as directed by the attending physician.. You will work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Observe and monitor patient conditions and perform OASIS assessments (If you don’t already know this assessment, we will teach you!) to develop an individualized care plan and adjust as needs change. Minimize Patient Risk: Administers medication as prescribed by the physician and helps decrease re-hospitalizations by prioritizing visits for high-risk patients. Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Educate the patient and family on the disease processes using teach back methods to ensure patient and family understanding. Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborates with the interdisciplinary team and physicians to ensure optimal care is provided. Rules and Regulations: understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors. We love to celebrate our team! • FUN and Friendly Work Environment - GREAT leadership team! • Supportive Staffing Ratios Key Responsibilities: • Assist residents with ADLs such as bathing, dressing, grooming, toileting, and feeding. • Take and record vital signs (temperature, pulse, respiration, blood pressure) as directed. • Assist residents with mobility, positioning, transfers, and use of assistive devices. • Observe and report changes in residents’ physical or emotional condition to nursing staff. • Answer call lights promptly and provide needed assistance. • Help residents with meals, including tray setup and feeding assistance. • Maintain accurate and timely documentation of care provided. • Keep resident rooms clean, organized, and safe. • Provide respectful, compassionate care in accordance with resident care plans. • Follow facility policies, state regulations, and infection control protocols. • Participate in in-service training and continuing education as required. Qualifications: • Valid STNA certification in the state of employment • High school diploma or GED preferred. • 2-3 Years of experience in long-term care or assisted living. • Ability to follow directions and communicate effectively with residents and staff. • Compassionate, dependable, and patient-focused. • Physical ability to lift, move, and assist residents and perform job-related tasks. • Current CPR certification (or willing to obtain upon hire).
