The primary role of this position will be to provide complex care coordination, emotional support and education to women experiencing pregnancies which have been identified as high risk. Complex care coordination will include telephone triage, patient portal management, result review and follow-up, diabetic care coordination, treatment plan and medication education, appointment scheduling, and administrative tasks associated with care coordination and management. The incumbent in this position will also be trained to the antenatal testing unit which provides non-stress tests, biophysical profiles, and related diagnostic testing.
