Schedule: Work schedule may include 8 to 12 hours per day with weekend, evening, night, and on-call coverage based on VNA needs. Staff are expected to cover for absences by rotating or working extra time. Staff are assigned to the service area of the VNA to meet patient/client needs. About the Job: Affiliated with Southern New Hampshire Medical Center Foundation Medical Partners is NH’s second-largest multi-specialty group Supports professional growth and a strong workplace culture Cardiology team delivers comprehensive heart care from prevention to advanced diagnostics RN Ambulatory role supports cardiovascular patients through collaborative care and education What You’ll Do: Provide direct nursing care to patients with cardiovascular conditions in an ambulatory setting Triage cardiac symptoms (e.g., chest pain, palpitations, shortness of breath) in-person and by phone Collaborate with cardiologists, APRNs, PAs, and other specialists to develop care plans Administer cardiac medications, vaccines, and treatments Assist with stress tests, EKGs, Holter monitor placement, and other cardiac procedures Perform point-of-care testing and collect specimens Manage lab and diagnostic follow-ups Educate patients on heart-healthy lifestyle, medication adherence, and chronic condition management Document assessments, interventions, and outcomes in EMR Monitor patients for changes and escalate care as needed Support safety protocols, infection prevention, and emergency response Participate in ongoing training related to cardiology care and EPIC updates Who You Are: Associate Degree in Nursing required; BSN preferred RN license in NH, NLC state, or MA required BLS/CPR certification required 1 year of nursing experience preferred Experience in cardiology, telemetry, or critical care a plus Strong cardiac assessment and clinical judgment skills Calm under pressure and effective in fast-paced environments Skilled in patient education and interdisciplinary collaboration Compassionate, detail-oriented, and team-oriented Proficient in EMR systems and Windows-based applications RN-BC, CCTM, or specialty certifications in cardiology or ambulatory care preferred Why You’ll Love Us: Health, dental, prescription, and vision coverage for full-time & part-time employees Short-term, long-term disability, life & pet insurance Tuition reimbursement & Nursing Student Loan Paydown Program 403(b) Retirement savings plans Continuous earned time accrual & more!
