This position is PRN field work as a Care Manager, assisting seniors with coordinating their care between multiple providers and agencies. Enable senior citizens to live comfortably at home and bring peace of mind to family members knowing their loved ones are cared for. Serve as the expert to assist client with obtaining providers to meet their needs. Visit clients in their homes to conduct initial assessments and re-assessments, write client care plans, recommend and coordinate changes in the level of care, assist clients with understanding available care options and connecting them with providers. If desired, be available to assist with homecare client in-home assessments Required Skills & Qualifications RN with current license to practice in Virginia Certification as CMC, CCM, or other Care Management certification, or the requisite experience to obtain Care Management certification. Previous experience working in Homecare, Assisted Living, Home Health, Skilled Nursing Facility highly desirable. Highly proficient communicator, comfortable with various communication methods including phone, email, Zoom, telehealth, etc. Reliable transportation and willing to travel to client homes and appointments.
