LVN Charge Nurse (Licensed Vocational Nurse) GENERAL DUTIES AND RESPONSIBILITIES: CLINICAL • Complete all required record keeping forms/charts upon the resident’s admission, discharge, transfer, etc. File in the resident’s chart and/or forward to the appropriate department. • Receive verbal orders from attending/alternate physician and transcribe the physician’s order sheet. • Initiate resident Plans of Care on admission. • Chart TPRs, blood pressures and blood glucose as required. • Order prescribed medications, supplies and equipment as established by the facility’s policies and procedures. • Review physician’s orders and transfer/preadmission information for completeness. • Prepare/administer medications as ordered by the physician. • Answer telephone inquiries as outlined in the facility’s established policies and procedures. • Chart licensed nurses’ notes in an informative and d3scriptive manner that reflects the care provided as well as the resident’s response to the care. • Forward new diet orders, and/or diet changes to the dietary department. • Transcribe physician’s orders to resident’s charts, Medication Administration Records, Treatment Administration Records and RNA sheets to assure that they remain current. • Verify the identification of the resident before administering medication or treatment. • Requisition and arrange for diagnostic and therapeutic services as ordered by the physician. • Assure that narcotic records are current at all times • Admit, transfer and discharge resident. SUPERVISION • Meet with unit personnel, on a regular basis, to assist in identifying and correcting problem areas, and/or the improvement of services. • Review and evaluate assigned personnel and make written evaluations to the RN Supervisor or Assistant Director of Nurses. • Coordinate Nursing service with other departments and nursing units. • Notify the attending physician, RN Supervisor and Director of Nurses when a resident is discharged without approval. Assure that appropriate documentation is made in accordance with the facility’s established policies and procedures. • Assure that a stock of medications, medical supplies, equipment, etc., is maintained at all times. Requisition equipment and supply needs as required. • Initiate investigation of accidents and unusual occurrences and make the necessary written report to the Director of Nurses as established in the facility’s policies and procedures. • Notify the resident’s attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident has a significant change of condition or becomes critically ill or injured, as outlined within facility’s established policies. • Work with the facility’s consultants as necessary and implement recommended changes as directed by the RN Supervisor of the Director of Nurses. • Investigate complaints and grievances and make necessary oral/written reports to the RN Supervisor or Director of Nurses. • In cases of death, notify the funeral home and ensure that the body is properly prepared/cleaned for pick-up. • Evaluate emergencies and determine emergency measures to be used, including the use of chemical or mechanical restraints. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Your Role on the Team Position Summary The clinical nurse collaborates with interdisciplinary teams to develop and implement a plan of care for a specific group of assigned patients, ensuring the coordination of care between other disciplines and support staff. Performs professional nursing duties that incorporate the psychomotor and age appropriate cognitive skills of the patient/family/significant other to assess, plan, intervene and advocate for the patient on an ongoing basis throughout the continuum of care. Will evaluate patient outcomes, effectiveness of plan of care, and readiness for discharge and revise plan or make referrals as necessary. Acts as a role model and mentor to a variety of students and nurses new to the unit. Functions as a (PCC) Patient Care Coordinator as requested by the supervisor/manager. Required Qualifications What It Takes to be Successful Minimum of one year of nursing experience in Perinatal Services All external candidates must have a Bachelor of Science in Nursing Ability to work variable shifts including evenings, nights, weekends, and holidays Ability to read, write and communicate effectively in English Ability to maintain flexibility and work well in a fast paced, constantly changing environment Ability to maintain a work pace appropriate to the workload AWHONN Advanced Fetal Heart Monitoring (required within 6 months of hire) Preferred Qualifications Understanding of applicable regulatory requirements Bilingual skills to communicate effectively with patients and families Licenses And Certifications Registered Nurse Neonatal Resuscitation Program Basic Life Support Advanced Cardiac Life Support Education Bachelor of Science, Nursing Total Rewards We offer a wealth of benefits to make working at UCI even more rewarding.
