Nurse Unit Manager Alliance at Coleman is a 45-bed non-profit skilled nursing facility with an exciting opportunity for an experienced Nurse to join our management team as the Nurse Unit Manager. We are looking for someone who is compassionate and can effectively communicate with residents, families, visitors, co-workers, physicians, and other employees. Ability to consistently demonstrate accountability, responsibility, and dependability. Able to communicate daily with nursing personnel providing clear, concise information, exercising independent judgment and critical thinking. Ability and willingness to work well with staff and to function as part of a team. · Works five days per week as designated. As needed, provides administrative supervision for the facility during weekend or for holiday coverage. · Ensures staffing to meet patient/resident needs · Develops assignments and schedules that combine the needs of the patients/residents with the capabilities of the staff. · Ensures response to unanticipated patient/resident needs. · Assists the scheduler in preparing monthly 24-hour nursing schedule for unit. Assure minimum staffing on unit and report all staffing problems to Director of Nursing Services. · Presents as a role model and acts to facilitate and develop the Modified Primary/ Team Nursing Model. · Coordinates nursing services with other resident care activities/services. Serves as clinical advisor in planning, implementing, and evaluating resident care. · Oversees accuracy and timeliness of all nursing documentation. Direct and organize required Medicare and Medicaid documentation. · Coordinates and participates in the orientation, education and evaluation of personnel as needed in cooperation with the Coordinator of Education and Infection Control. · Review the performance of and establish performance agreements with the nurses and the nursing assistants per facility policy. · Serves as liaison between nursing staff on unit and other departments. Interprets Human Resource policy as needed for the nursing staff. · Participates in interviews and hiring decisions of nursing staff for unit. · Ensures that nursing/medical supply needs for the units are met. · Assumes direct patient/resident care responsibilities as needed. · Administer Mantoux test to patients/residents on unit. · Administer Mantoux test to staff members as requested by the Coordinator of Education and Infection Control. · Performs PI monitoring activities as established by Nursing Department and PI Council. Ability to direct and implement promotion of PI concepts. · Make daily nursing rounds on all patients/residents of unit. Communicates with LPN Nurse Managers and makes daily rounds of their units. Make rounds with Physicians as possible. · Participate in Interdisciplinary meetings, resident/patient family conferences, discharge planning, and other meetings as needed. · Ensures that incident reports from resident/patient, staff and visitors are accurately completed and filed timely with administration. · Edit monthly physician orders/MAR/Treatment records with Staff Nurses. · Assists in maintaining safety and cleanliness of the work area. · Provides input in the formation and evaluation of standards of care. · Provides resident/patient and family teaching as needed. Documents important information on the agency’s “Request for Service” form creates a case in Forcura and the current EMR. 3. Attempts to assess if the potential patient can manage safely in their home and assists referral source in making decisions regarding safe discharge planning. 4. Assesses if the patient is eligible for home health services under their payment source . Try to establish Home Bound status and if medically necessary/reasonable requests. 5. Effectively manages HHVNA and Hospice referrals for patients who do not have insurance coverage for requested services and consults with other RN s as needed. Communicates the mission of the agency to provide care for patients regardless of ability to pay, and manages expectations about the available services. 6. Provides information of other community resources to the referring party if needed and if patient does not meet home care criteria. 7. Communicates with billing to verify atypical insurances as requested by referral sources and to support UR and Clinical Staff. 8. Identifies pre-admit evaluated patients and works to determine home care eligibility and appropriateness for home care . Communicates with department manager to determine acceptance of referral. 9. Follows referral department guidelines regarding orders required to initiate home health services . All physician orders/approvals are noted in the referral orders. 10. Prioritizes completion of referrals based on clinical priority, patient safety, and insurance precertification. 11. Effectively manage the time it takes to complete referrals by understanding what information is desirable vs necessary. 12. Uses critical thinking and problem - solving skills to resolve referral issues limiting the need to involve the referral source. 13. Utilizes clinical judgment to determine optimal services to manage a patient safely in the home setting. 14. Adheres to referral order documentation guidelines developed to guide agency initiatives. 15. Maintains productivity standards as defined by the department standards. 16. Obtains prior authorizations and pre-certifications as required by insurer . 17. Completes e-tracking and Forcura case final verification to inform clinical departments on admission state, diagnoses and priority of ordered care. 18. Keep the Intake clinical manager informed of any problems communicated by referral sources. Report any changes or trends noted in referral patterns to Department manager. Keep abreast of third-party regulations and insurance companies and HMO’s. Identifies opportunities to generate potential referrals for all agency services to meet the care needs of the patient. 19. Demonstrates required knowledge of Home Health Foundation services for which a patient may be eligible. 20. Selects the correct referral sources in the referral source tab to enable accurate data collection. 21. Keeps department Clinical Lead informed of issues related to workflow. 22. Reaches out to other RN s and/or Intake Clinical Manager to determine if a referral is to be accepted or declined. 23. Reports Compliance concerns to the CEO or Chief Compliance Officer when applicable. Physical Requirements : 1. JOB SUMMARY: Under the direction of Management, Physician and/or Nursing, incumbent is responsible for assisting with patient care and medical office clinical support and clerical functions. Assists with development and implementation of quality assurance program and trains staff for infection control, nursing, pharmaceutical, ancillary and laboratory services. Attends treatment planning conferences and professional staff meetings. Available for 24 hour coverage for consultation and emergencies. Minimum of three (3) years of nursing experience. Valid Registered Nurse license issued by state of employment. Nursing degree from an accredited RN program, Associates in Nursing or Bachelor of Science in Nursing. Ability to work independently. Excellent customer care skills. Excellent communication skills. At least 2 years' experience in substance abuse/recovery program preferred. CPR/First Aid Certification is required for this position.
