Position Responsibilities: · Demonstrates ability to initiate and safely administer antineoplastic agents. · Demonstrates clinical skills and judgment in caring for cancer patients and end of life issues. · Demonstrates the ability to safely care for patients receiving internal and external radiation treatments. · Demonstrates the ability to care for neutropenic patients. · Educates patients, families, visitors and ancillary staff on neutropenic precautions. · Demonstrates ability to access and care for patients with implantable venous access device (por-a-cath). · Demonstrates safe and correct calculation of antineoplastic agents. Including, but not limited to, calculating body surface area (BSA), Absolute Neutrophil Count (ANC). · Provides Pre and Post Operative care to patients undergoing mastectomy, lobectomy, prostate seeding and insertion of tandem and ovoid. · Demonstrates ability to safely care for post craniotomy patients. · Demonstrates ability to identify signs and symptoms of common oncology emergencies. · Levels II/III: Evaluates, assesses and documents patient response to chemo and radiation therapy. · Levels II/III: Provides ongoing observations, interventions and documentations on patients receiving high dose Interleukin II. · Levels II/III: Coordinates, consults with oncology pain management team. · Level III: Plans and presents unit in-services on new information relating to cancer diagnosis and treatments. · Level III: Consistently serves as preceptor for new staff members. · Level III: Acquires increasingly complex clinical skills in identifying, intervening and documenting signs and symptoms of common oncology emergencies. · Successfully challenges EKG I test upon hire, if previously trained or successfully completes EKG I program within 90 days of hire. · Successfully interprets life-threatening dysrhythmia and subsequent treatment. · Demonstrates safe and correct defibrillation. · Administers anti-dysrhythmic therapy as ordered. · Demonstrates basic understanding of patient's daily status, including reason for admission, surgeries/procedures performed, pertinent diagnostic data, learning needs of the patient, and discharge plan. · Demonstrates understanding of patient multi-system complications, evidenced by identification and reporting of abnormal findings to appropriate physician in a timely manner and implementation of nursing intervention. · Performs initial vital signs and admission history to include introduction to patient and family. Position Summary The PRN Nurse Practitioner provides services including assessments, clinical evaluations, medication management, patient education, and collaboration with the interdisciplinary team. This role supports home health patients with chronic conditions, acute changes, post-hospitalization needs, and ongoing follow-up care. The NP will serve as a virtual and in person clinical resource, ensuring timely and appropriate care while assisting in preventing unnecessary ER visits and hospital readmissions. This is an as-needed position to provide flexibility and coverage for patient needs, high census periods, and staff PTO. Key Responsibilities Conduct virtual patient assessments (audio/video) to evaluate symptoms, chronic disease status, medication needs, and overall well-being. Provide timely clinical recommendations, interventions, and treatment plans aligned with home health goals. Support transitions of care by completing post-hospital and post-procedure follow-up visits. Provide education on medication adherence, disease management, and self-monitoring strategies. Empower patients to manage their health between in-person nursing visits. Collaborate closely with home health nurses, therapists, social workers, and physicians to support the patient’s plan of care. Assist in facilitating referrals, orders, and escalations as needed. Document all encounters, assessments, recommendations, and communication in the EMR per company policy. Ensure documentation meets Medicare/Medicaid and regulatory standards. Participate in virtual team meetings or case reviews when available. Qualifications Current Nurse Practitioner license and certification (FNP, AGNP, or relevant specialty) in the state of FL. A minimum Master of Science in Nursing or Doctor of Nurse Practice from an Accredited University. 1–2 years NP experience in primary care, family medicine, urgent care, home health, or telehealth. Strong clinical assessment and triage skills for virtual care settings. Proficiency using telehealth platforms, video communication tools, and EMR systems. Exceptional communication skills with the ability to deliver clear, compassionate guidance remotely. Reliable high-speed internet and private environment for virtual visits. Experience with home health populations, chronic disease management, and transitional care. Knowledge of Medicare/Medicaid regulations and home health documentation standards. Position Responsibilities: Management of Payor Processes and Relationships Develops and maintains effective relationships with payors, including contact as required with representatives from payors to ensure accomplishment of utilization management activities Oversees the management of observation cases, both concurrently and retrospectively, as needed. Supports the clinical denial management process, ensuring that all denials related to medical necessity and delays in service/discharge are identified and addressed promptly and documented appropriately Ensures concurrent flow of information between the Care Management Department and other MSMC departments responsible for managed care contracts and patient accounts/billing Participates with other MSMC entities who are involved in the denial management process to ensure that denials and appeals are tracked, trended, and reported to the Director regularly Is familiar with Payor policies and appeals processes and ensure appeals are completed and followed up on in a timely manner Department Data and PI Generates and interprets reports/conducts audits to ensure compliance with documentation and to identify opportunities for improvement; follows up on any areas/individuals identified as having room for improvement Collaborates with other departments to generate outcome data as needed to measure and trend performance indicators identified in the Utilization/Medical Management Plan. Ensures the management of data collection, aggregation, and dissemination across the continuum, including: clinical, financial, and operational performance; this includes concurrent and retrospective medical record review Staff Development Participates in hiring, reviews, discipline, and grievance handling to ensure optimal individual and team performance. Participates in staff development activities, interacting as needed with the director, customers, and others to continuously improve competency and effectiveness of the department. Identifies and addresses skills or behaviors requiring improvement, both at an individual level and at a department level. Provides coaching as needed to optimize performance and ensure safe and satisfactory care to patients and optimal customer service. Other Departmental Functions Supports Director in development of department operational budget. Analyzes costs, identifies/develops strategies to ensure compliance with budgetary constraints and provides justifications for budget variances. Provides daily oversight and management to payor teams; Ensures that all of the functions of the staff are accomplished Managerial Behavior and Development Engages in self-development in clinical, professional, and managerial areas; successfully completes leadership development curriculum. Adheres to standards, policies and procedures at both the hospital and Departmental level. Promotes a service orientation in the performance of position duties and responsibilities and in interactions with patients, hospital staff and visitors.
