Communicate with physicians, patients, and the family members of patients concerning patient care. Collaborates with providers, and practice staff in identifying appropriate patients for care management. Perform other job related duties as assigned. As well, provide regularly scheduled in-service education programs and opportunities for continuing education for the staff which covers the physical, emotional, spiritual and social needs of hospice patients and their families during the final stages of illness and during dying and bereavement. Performs other duties as assigned including those within the scope of current, valid professional licensure/credentials (i.e. RN, SW, Chaplain, MT, HA, etc.).
