Position Summary: We have an exciting opportunity to join our team as a Per Diem Nursing Attendant-Centralized Nursing Service-23 Hours Per Month. In this role, the successful candidate Assists in implementation of the nursing process by performing direct and indirect patient care to assigned patients under the supervision of the Registered Professional Nurse. Assists in maintaining an orderly and safe patient care environment and unit. Job Responsibilities: Assists with Admission/Transfer/Discharge of patients: Orients patient to hospital environment. Provides patient with toileting supplies. Assists patient to remove clothes, complete property check list and safeguard personal property through Security. Assists in transfer of patients by gathering belongings, completion of clothing list and assisting in moving the patient. Prepares bedside unit to receive patients. Discharges: Assists patient to dress and pack personal belongings. Escorts patient as necessary and remains with the patient until he/she has left the building. Conducts patient rounds for toileting, positioning, comfort, safety and personal needs. Answers patient call lights and responds to patient needs promptly. Assemble O2 (e.g., nasal cannula) and suction set- ups. Provides basic hygiene to patients: bathing, AM/PM care, mouth care, denture care, hair care, nail and foot care, shaving, assists patients with bedpan, urinal and/or commode. Prepares patients for meals including: Clearing overbed tables and placing them within reach of the patient. Positioning patient correctly for eating/tube feedings. Assisting patients by preparing utensils and food items. Feeding patients. Performs and/or assist patient with simple treatments. Remove and reapply SCD wraps as directed by the RN. Assists RN in enema procedures. Collects specimens as directed by RN. Informs RN of any audible clinical alarms such as IV pumps, feeding pumps, ventilators, bedside monitors Assists with moving, lifting, positioning and transporting of patients. Assists with removal and security of patient personal items prior to procedures, as needed. Changes linen in accordance with policy and as necessary. Takes and electronically records blood pressure, temperature, pulse, pulse oximetry, and respiration per care level standard and as directed by the RN. Reports abnormalities to RN. Weighs patients and electronically records on admission, as ordered, and/or as directed by RN. Measures and records oral intake, urinary output, bowel elimination, gastric and other drainage onto I&O record (form or electronic). Calculates 8 hour totals. Empties drainage bags. Performs post mortem care. Performs safety observations for patients on continuous observation or in restraints, per policy and as directed. Reports any changes observed in the patients condition to the RN. Responds to and meets patient/family needs and customer concerns as soon as possible with compassion and understanding. Established effective, caring relationships with staff, patients, family and other members of the health care team. Maintains competence through in-service and educational programs. Attends staff meetings and reviews/signs minutes of meetings not attended. Maintains a safe and uncluttered environment of care in patient rooms including removing excessive furniture, supplies and equipment. Assists with supply and equipment management of patient, procedure and supply rooms per department standards. Maintains cleanliness of utility rooms and pantries as assigned. Performs other duties such as errands, relieving other units, bringing stretchers and assisting other members of the health team, as directed. Graduate of Certified Nursing Attendant program or minimum 6 months relevant experience. BLS or Heartsaver CPR/AED certification. Description The (Per Diem) Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Description The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of inpatients to determine the appropriateness of the admission and continued stay, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities Principle Duties and responsibilities include, but are not limited to: Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. Documents all clinical reviews in Canopy. Supports the mission, vision, philosophy and goals of the Medical Center. Case Management: The CM process will include: Assessment of the patient’s clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. Identification and documentation of variances affecting the LOS and the discharge planning process. Conducts follow-up of any delays in treatment or reporting of results. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. Documentation of outcomes achieved and identified internal and external barriers. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. Utilization Review: Maintains a working knowledge of the UR requirements of each payor within the patient population; Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. Provides clinical information requested by the managed care companies in accordance with contractual agreements. Works collaboratively with physicians and managed care companies on concurrent denial appeals Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. Discharge Planning: Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). Obtains authorizations from managed care companies for post-discharge services. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. Liaison with financial department for current insurance coverage. Collaborates and participates in the appeals process with all members of the interdisciplinary team. Collaborative Relationships: Develops and maintains effective working relationships with interdisciplinary team and Case Managers within managed care organizations. Fulfills clerical responsibilities as assigned which may include: sending and receiving patient medical records; obtaining lab/x-ray reports, hospital notes, referral information, etc. Completing forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. These responsibilities may be carried out using the electronic medical record (EMR) system or manually as necessary. Enters orders into the electronic medical record (EMR) system or prepares prescription refill requests on behalf of the physician for the review and approval by the ordering physician. Inventories, orders or re-stocks medical supplies as applicable. Provides set-up of examination room and instruments in accordance with proper sterilization techniques established by hospital and infection control policies. Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, crash cart, point of care testing or others as needed. Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy. Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations. Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances. Assists in the maintenance of medical charts (filing, Op Reports, test results, home care forms). May schedule surgeries/procedures or work in conjunction with Surgical Coordinator verifying times with patients; preparing charts, pre-admissions and consent forms as necessary. May be required to perform proficient phlebotomy duties. May perform appointment scheduling and registration for patients as well as updating patient demographic and insurance information. May maintain, adjust and confirm patient office schedule. May check and verify & update insurance information utilizing internal or web sources. May perform and verify pre-certification for procedures as necessary. Performs other duties as assigned.
