Job Description The Registered Nurse in the Sexual Health Center provides direct patient care, coordinates treatment plans, collaborates with interdisciplinary teams to ensure quality outcomes, maintains clinical documentation, adheres to safety protocols, and supports patient education and advocacy. The RN will work partly in the ambulatory fixed site at 200 South Street in Morristown, partly in the Morristown Medical Center Emergency Department, and partly in the community in an Atlantic Mobile Health vehicle and at community-based organizations through Northern New Jersey including, but not limited to, Morris, Sussex, and Passaic Counties. The Sexual Health Department in a small, interdisciplinary team that primarily promotes public health initiatives funded through grants and awards. Public health initiatives include prevention, testing, and treatment for HIV, HCV, and sexually transmitted infections; harm reduction health programming such as preventative care, acute and chronic disease management, and wound care; education and counseling to promote safer sex and use strategies. Principal Accountabilities Patient Assessment: Perform systematic, comprehensive, and accurate assessments. Assess potential discharge needs during the initial assessment process. Administers medications to patients and monitor effects of medications and treatments and report adverse effects in a timely manner. Assess patient for pain level. Provide appropriate intervention, reassessment, and documentation. Systematically monitor the maintenance of protocols, standards, and plans of care. Plans and provides individualized patient education. Clinical Skills: Perform nursing-related skills and activities competently. Analyze situations critically. Maintain complete and accurate documentation. Identify and document realistic long and short-term goals. Complete interdisciplinary plan of care record. Participate in interdisciplinary rounds. Evaluate the effectiveness of interventions and revise the plan of care as needed. Recognize and evaluate patient and family responses to therapies and nursing interventions. 3: Professional Skills: Communicate clearly and effectively with all members of the healthcare team. Complete hand-off communication and give complete reports to other caregivers. Demonstrate critical thinking and time management skills necessary for effective patient care. Assume responsibility and accountability for the safe nursing care of assigned patients. Delegate responsibilities appropriately to others. Conducts regular patient rounds to ensure safety and enhance patient experience. Actively participates in daily interdisciplinary huddles to support coordinated care. Utilizes approved clinical communication tools in accordance with organizational standards. Quality and Safety: Adhere to OSHA, Joint Commission, Standards of Nursing Practice supported by NJ BON, professional association, ANA Code of Ethics & Scope of Standards and Practice NJ Board of Health, and hospital standards. Use two patient identifiers and check allergies before medication administration. Follow hand washing protocols and maintain isolation precautions. Properly use and store equipment, checking expiration dates. Demonstrate leadership and function as a resource/mentor. Actively participates in Shared Governance. Outreach and Education: Participate in local, state, and national public health planning initiatives. Assist with monitoring and evaluation of grant-funded projects. Provide education to communities, patients, and families in accordance with grant requirements. Attend external meetings and field-based outreach events. Performs other related duties as assigned. Other Requirements Available to work at least one evening per week and one Saturday per month Independent travel to field-based locations and trainings or meetings within the State of New Jersey (business mileage reimbursement provided) Ability to work indoor, outdoor, and mobile flexible environments Lifting/carrying up to 50lb Required QUALIFICATIONS Graduate of an accredited Nursing Program. Current state licensure as a Registered Nurse. BLS certification. Preferred Bachelor’s degree in nursing. All ASN must complete their BSN studies within five years of their start date. Master’s degree in nursing or related field. 1+ year of experience in Nursing or relevant area. ACLS certification. Description RN/Case Manager Mount Sinai West Case Management FT Weekdays/Weekends/Holidays The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of inpatients to determine the appropriateness of the admission and continued stay, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities Principle Duties and responsibilities include, but are not limited to: Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. Documents all clinical reviews in Canopy. Supports the mission, vision, philosophy and goals of the Medical Center. Case Management: The CM process will include: Assessment of the patient’s clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. Identification and documentation of variances affecting the LOS and the discharge planning process. Conducts follow-up of any delays in treatment or reporting of results. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. Documentation of outcomes achieved and identified internal and external barriers. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. Utilization Review: Maintains a working knowledge of the UR requirements of each payor within the patient population; Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. Provides clinical information requested by the managed care companies in accordance with contractual agreements. Works collaboratively with physicians and managed care companies on concurrent denial appeals Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. Discharge Planning: Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). Obtains authorizations from managed care companies for post-discharge services. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. Liaison with financial department for current insurance coverage. Collaborates and participates in the appeals process with all members of the interdisciplinary team. Collaborative Relationships: Develops and maintains effective working relationships with interdisciplinary team and Case Managers within managed care organizations. Participates in nursing orientation. Interfaces with other departments in the Medical Center. Qualifications Bachelor’s Degree in Nursing; Masters preferred. Previous experience as in homecare, long term care or utilization review preferred. Discharge Planner or Case Manager preferred. Manager or a minimum of 3-5 years’ experience as a RN in an acute care setting. Current NYS RN nursing license. CCM certification preferred. Re-credentialing of CCM status expected.
