After that, caregivers will work 3 12-hr shifts until the 12th week. A caregiver who excels in this role will: Provide supervision, assessment, planning and direct patient care to hospice patients. Perform accurate and complete admissions assessments and re-assessments. Assess patients for safety risks, abuse and neglect. Assess, document and manage patient’s pain. Prioritize and document patient problems. Administer medications and treatments safely and accurately with a focus on patient comfort. Coordinate care for managing a caseload of patients. Document changes in patient condition and effectiveness and timeliness of care/interventions. Evaluate, update and revise plans of care. Assess educational needs of patients, caregivers and family while considering cultural issues and other special challenges. Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs.
