JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Once an offer of employment is made, it is contingent upon satisfactory references, as requested, and criminal background checks by regulation. 6. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 7. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills. 8. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. 9. Per the guidance of Axia Women’s Health patient protocol and care center physicians, the Certified Medical Assistant professionally engages Axia Women’s Health patients by assisting with patient care, recording critical patient information, and providing information to patients so that they may fully utilize and benefit from our clinical services. Essential Functions: Greet and acknowledge patients in a professional and courteous manner. Escort patients to exam rooms and ensure they are comfortable. Prepare patients for examinations, complete comprehensive clinical intake. Maintain accurate and detailed patient records. Perform and document accurate blood pressure, height and weight, and patient temperature. Demonstrate appropriate chaperone technique and documentation. Assist with patient examinations and office-based procedures (Routine procedures performed during the regular course of the day including but not limited to colposcopy, IUD insertions, and endometrial biopsies). Administer medications and injections as ordered by providers. Collect and prepare laboratory specimens. Computerized physician order entry (CPOE) – Enter and send medication and test orders per provider instructions. Clean, disinfect, and sterilize medical instruments and ensure exam rooms are clean and stocked. Provide information regarding procedures, medications, and follow-up care. Respond to patient inquiries both in person and over the phone. Assist with scheduling appointments and follow-up visits. Ensure patient safety within clinical settings. Work collaboratively with physicians, providers, nurses, and other staff to ensure efficient practice operations. Upon successful completion of competency skills training, assist in office-based surgical procedures (Anything historically performed in an Operating Room and/or requiring moderate sedation or higher including but not limited to LEEP, operative hysteroscopy, ablations, and D&Cs with or without suction) by preparing the procedure room, ensuing the availability of necessary equipment, providing support to healthcare providers during the surgeries, and post-surgical processes including disinfection. Safely and efficiently assist patients with mobility, including transferring them between examination rooms and treatment areas. Ensure compliance with all Standard Operation Procedures (SOP’s) and policies (including HIPAA & OSHA). Participate in team meetings and contribute to quality improvement initiatives. Adhere to practice policies, procedures, and protocols. Demonstrate commitment to the organization’s mission, vision, and values by embodying its principles in daily activities. Education & Experience: High School diploma or equivalent required. Current national certification as a Medical Assistant, such as CMA, RMA, NCMA, CCMA required. CPR Certification required if performing Office Based Surgeries OB/GYN or Women’s Health experience preferred. eClinicalWorks (EMR) experience preferred. Pay or shift range: $18 USD to $23 USD The estimated range is the budgeted amount for this position. IMT participants interact with homeless services, criminal justice, and behavioral health service systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served. Registered Nurse-Intensive Mobile Team-Full Time Salary: $107,120 plus benefits Intensive Mobile Treatment (IMT) is an innovative, creative, trauma-informed mobile practice model designed to provide treatment, rehabilitation and support services to individuals whose needs have not been met by traditional services. IMT participants interact with homeless services, criminal justice, and behavioral health service systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served. IMT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, criminal justice, peer support and behavioral health recovery. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant's choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week. Overview The Registered Nurse (RN) functions as a primary practitioner who assesses and monitors IMT participants' clinical health and response to treatment. The nursing approach of assessment, planning, intervention, implementation and evaluation is used to assists individuals in their achievement of mental health, rehabilitation and recovery goals. Core Principles The job responsibilities of all staff extend to understanding and incorporating certain principles into their work and into their relationships with program participants. These principles are: Program participants' right to self determination; Respectful communication; Services that support recovery and healing consistent with and nurturing each participant's cultural background, experience, identity, and values. Clear professional boundaries to support the limits and possibilities of services. Essential Job Functions Develop, implement and maintain IMT program nursing standards. Guide the development of policies and procedures and identification of resources essential to delivery of nursing services. Conduct psychiatric, physical health and other assessments with a special focus on conducting assessments and evaluations with individuals using substances or experiencing street homelessness. Develop, or consult and coordinate with team to develop, plans to address participant health needs. Monitor and modify treatment plans as indicated by participant responses and conditions. Use online databases and portals e.g. PSYCKES, Healthix, MAPP, and others to support assessment and treatment planning. Order, interpret and evaluate medical diagnostic tests to identify and assess participant conditions. Provide management and administration of medication in conjunction with the psychiatrist. Provide a range of treatment, rehabilitation, and support services. Utilize motivational interviewing techniques and a trauma informed approach when delivering individual and group based health services. Administer IMs and vaccines, plants and reads PPD's, and performs phlebotomy as per orders Monitor vital signs and side effects of medications and reports findings to team, and within annual health assessments. Collaborate with psychiatrist and other medical personnel to schedule appointments and coordinate care. Make referrals to community physicians and work jointly with hospital and institutional staff. Intervene and advocate on participant's behalf with, for example, collaterals, agencies, facilities and family members; assist with problem solving to help participant access needed supports. Provide consultation to IMT team regarding participant medical issues; serves as a resource to the team on medical concerns. Maintains accurate, detailed reports and records including required documentation, progress notes and health related data, in AWARDS. Evaluate and establish system for reporting on nursing intervention and expected outcomes. Attend and participate in supervision, meetings and training sessions as required. Provide 24-hour crisis intervention services on rotating basis. Job Qualifications Commitment to person-centered treatment strategies, upholding participants' rights, and self-determination in service provision. Currently licensed as a registered professional nurse by the New York State Education Department. Minimum two (2) years' experience working as an RN. Minimum two (2) years' experience working with persons with mental health condition in health or social services setting. Excellent written, verbal and interpersonal communication skills. Ability to synthesize and summarize information and make judgments regarding care. Eligible for full and unconditional participation in the Medicaid and Medicare programs. Computer proficiency in Windows operating systems and programs, such as MS Word, Excel, PowerPoint, as well as comfort with learning new electronic systems. Comfort providing treatment in non-traditional settings, including but not limited to the street, community spaces, and shelters.
