Summary: Under the direct supervision of a Registered Nurse/licensed Nurse, this Job will assist with routine and repetitive patient care activities in a nursing unit. This Job also performs specific clerical, organizational, and patient-focused activities. The patient-focused tasks will be supportive in nature. Assists with the admission, transfer, and discharge of patients. Completes pertinent documents. Prepares rooms for admission by setting up IV pots, and adjusting bed to the appropriate level. Maintains security/confidentiality when transporting patients and interacting with family and visitors. Cleans, procures and returns equipment and supplies. Run errands for the unit as required. Assist Licensed personnel in providing a safe and comfortable environment at all times; replenish patient room with water, linen, and towels, as appropriate. Perform unit-specific patient care tasks in accordance with the skills checklist and documents on the flowsheet. Identify obvious changes in the patient's condition and take responsibility for reporting those changes promptly to the nurse in charge. Assist with the ordering and maintenance of stock items according to established inventory when needed. Keeps unit supplies in an organized and orderly fashion. Respond appropriately to emergencies. Communicate effectively with other team members during and between shifts. Responds to phone calls and patient call lights. Appropriately adapt assigned patient assessment, treatment, and/or care methods to accommodate the unique physical, psychosocial, cultural, age-specific, and other developmental needs of each patient served. Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the population-specific and developmental needs of patients served by the department. Registered Nurse Transfer Center Lead - Patient Intake Transfer Center

Summary: The objective of the access center is to provide all outlying regional healthcare providers with easy access to the many physicians, specialties, and services. This position within the Transfer Center is responsible for the clinical review, triage, prioritization, and coordination of inter-hospital transfers to hospital. The RN in the transfer center will review clinical information to assist the sending physician and facility with obtaining the appropriate physician or specialty. Demonstrates knowledge of disease process, normal vs abnormal clinical findings, and usual and customary healthcare treatment. From the clinical information, the RN will determine level of care and a plan of care will be initiated. The RN will also review interqual criteria to ensure that all admits and transfers are appropriate and determine the status of each admission. This position will be directly responsible for initiating several critical clinical protocols, for example, Code STEMI and Code Stroke. The RN will coordinate all activities for these protocols including communication with physicians, appropriate interventional teams, and transportation, whether ground or air. This position will work within day to day operations of Bed Control at the Hospital. This position will serve as an information resource and liaison between physician requests for admission and/or transfers, nursing departments, and other hospitals requesting transfers in. This position will make bed reservations, assignment of beds for patients and coordinate with the nursing floor and housekeeping to maintain adequate patient throughput. This position will be the primary access point for physicians to direct admit to facility. After obtaining the clinical information and orders from the physician, the RN will assess the appropriateness of the admit and determine what status the patient should be by using interqual. The RN will manage all transfer related issues and collaborate with the Clinical and associate vice president regarding daily clinical leadership for the unit. Possesses exceptional customer service skills and fosters positive physician relationships. Serves as a staff role model by providing leadership that encourages teambuilding, communication, positive attitudes and change. Supports the practice of nursing by modeling best nursing practice and adherence to ANA standards of care. Assists with compliance to safety, environmental, infection control, quality improvement and other regulatory standards. Ensures patient safety during clinic visits, per hospital policy. Maintains a clean clinic working environment. Implements standard and transmission-based precautions, per hospital policy. Performs hand hygiene, as outlined in hospital policy. Reports any safety hazards or violations in patient or clinic environment to clinic manager or designee. Participates in the quality improvement process, including QAPI projects and EOC rounds. Participates in the clinic preparation process by gathering appropriate clinical documentation for review by the medical provider/licensed practitioner. May operate equipment including computers, printers, scanners and fax machines. Obtains medical records from outside facilities when needed or requested by medical provider. Scans outside correspondence into the appropriate area of the electronic medical record. Answers the telephone and routes calls promptly to the appropriate person.
