Community Care Home Health Services, a fast growing organization is looking to hire Per Diem RN's for all our locations (Rochester, Buffalo, White Plains, NYC, Smithtown) on a fee for service schedule. We need Per Diem RN's for any day and shift. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. You will be compassionate and cool-tempered. You will be able to follow health and safety guidelines faithfully and consistently. The goal is to promote patient’s well-being by providing high quality nursing care. DUTIES: • Schedule and conduct face to face comprehensive clinical assessments and provide care for potential clients according to state guidelines, and policies, procedures, and protocols as described by Community Care Home Care. • Conducts comprehensive assessment of all aspects of potential new members condition - clinical, social, and environmental to determine the level of services needed to appropriately keep member safe in the community • Gathers clinical information from various sources to address all conditions, and follows appropriate guideline, policy, procedure and clinical judgment in completing assessment requirements • Communicates with consumers, families, providers, and other parties as needed to complete accurate comprehensive assessment • Understands the needs of vulnerable populations who have chronic or complex bio-psychosocial needs • Conducts all assessment functions in compliance with regulatory and company policies and procedures REQUIREMENTS: • Must hold valid Registered Nurse (RN) license. • Proficiency with computers is required • Effective communications, organizational, and interpersonal skills required • Ability to work independently is required • Effective computer and English language skills required • MLTC/ Managed Medicaid experience preferred **Community Care Home Health Services is committed to ensuring the safety and wellbeing of our patients and team members, so we may continue serving our communities by delivering exceptional care. Part Time Wound Care Nurse Practitioner

While an interdisciplinary care team provides medical care to patients in long-term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population. We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities. WOUND NP ROLE AND RESPONSIBILITIES: Provide wound care services for long-term care patients residing in skilled nursing facilities. Prescribe appropriate medications, including controlled substances in accordance with NH Law. Order, review, and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with the primary care physician. WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of New Hampshire. Board Certification. DEA licensure (Optional) Clinical background in adult, family, or geriatric specialties. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Ability to work with an EMR. Ability to work within an interdisciplinary care team to coordinate patient care.
