Registered Nurse, Case Manager Job Responsibilities: Assesses patients’ continuum-of-care needs by conducting comprehensive chart reviews to identify medical and social factors, and by interviewing patients and significant others to determine home care needs and existing services. Plans continuum of care to meet identified needs in collaboration with the healthcare team, with comprehensive knowledge of community resources and services. Implements care plans and coordinates resources to achieve identified goals, including referring patients to appropriate providers, serving as a liaison between patients, the healthcare team, and external partners, and assisting patients in navigating the healthcare system. Monitors and evaluates the effectiveness of the continuum of care plan by collecting data for outcome measurements such as readmission rates. Performs utilization management functions such as reviewing all admissions and continued stay of all patients to assure medical appropriateness of level of care and length of stay. Registered Nurse, Case Manager Education and Experience Requirements: Education: Graduate of an accredited school of nursing required (BSN preferred). Current CT State license required. CCM preferred. Experience: Minimum of three years nursing experience in either a hospital or homecare setting required. Membership and participation in professional organizations preferred. Current AHA Healthcare Provider CPR certification or equivalent within 3 months of hire required. Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care. Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaches behavioral health philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Initiates, supervises, and documents observance of the behavioral health aide and LPN delivery of care per agency policy. Informs supervisor and/or facility supervisor of unusual or potentially problematic nursing facility issues. Provides appropriate support at time of death and perform bereavement assessment. What is Required? High School Diploma or GED Current, unrestricted state RN licensee 1+ years of experience in a clinical care setting as a nurse Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Able and willing to travel within branch/office coverage area. Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes. Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care. Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate. Implements the plan for patient safety, using the patient, family, and community resources. Assists the patient in learning appropriate self-care techniques while delivering quality patient care. Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager. Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions. Documents medication regimen and updates medication profile according to established procedures. Treats patients and caregivers in the highest and most effective manner. Implement all available actions to prevent avoidable hospitalizations and ER visits. Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel. What is Required?
