Nurse Practitioner - Complex Care Services Department Summary Advanced Practice Registered Nurse (APRN) to function in the inpatient Complex Care Service (CCS) at Boston Children’s Hospital. CCS is a long-standing and nationally regarded inpatient service in Pediatric Hospital Medicine within the Division of General Pediatrics that serves hospitalized children with medical complexity, who have complex chronic conditions and functional limitations. Common reasons for admission to CCS include infections (aspiration pneumonia, pyelonephritis), chronic condition exacerbations (epilepsy, digestive dysmotility), and post-operative recovery following surgery (baclofen pump placement, gastrostomy, spinal fusion, etc.). This role will provide patient evaluation, assessment, and facilitate care for the patients on the CCS service and will implement interventions tailored to improve systems for patient care and enhance patient safety for this population. The CCS APRN will work collaboratively with supervising physicians, pediatric trainees (residents and fellows), and the entire nursing team to promote optimal clinical care. This APRN will act as a clinical resource, facilitating staff educational opportunities. The setting is fast-paced and collaborative with many opportunities for learning. Requirements include completion of a master’s degree program in Nursing. Clinical experience with children with medical complexity is preferred. Provide brief behavioral health interventions in the form of Individual, Family, or Group treatment. Possesses knowledge of substance abuse assessment, recovery principles, behavioral activation, and chronic disease self-management. 6. Complete intake of high-risk clients, working with the client, family and other members of the healthcare team as needed, to assess client’s behavioral health needs, physical health, support system, financial resources, and available community and government resources. Refer to crisis services as needed. 7. Determine with the client specific objectives, goals, and measures that meet the client’s needs and that have been identified through assessment. Utilize the needs assessment to find a mental health home for the client and provide warm hand-offs to long-term mental health providers. 8. Serve as an essential link between clients and all other care providers. Represents client/family by intervening, negotiating and promoting their concerns. Problems requiring advocacy may include social and health inequities or inadequate and non-existent hospital and/or community resources (i.e., insurance benefits, housing, transportation, etc.) 9.
